Propionic acidemia (PA) is an interesting differential diagnosis. It is estimated that forty infants are born with this condition each year in the US. (HRSA, 2023). PA is a genetic condition that prevents the body from breaking down certain proteins, fats, and cholesterol. This causes waste and toxins to build up that turn into acids. The blood becomes more acidic and leads to acidemia. (HRSA, 2023). Symptoms most commonly develop in the first week of life. These symptoms include vomiting, lethargy, hypotonia, failure to grow and gain weight at the expected rate, and dehydration. (Philadelphia, 2022). I agree with you. This infant has most of the signs and symptoms of propionic acidemia. Therefore, he may be in fact suffering from this condition and will require immediate treatment. Treatment of this condition follows a low protein and certain fats diet which requires a specific formula for infants. They will be under chronic treatment by a metabolic specialist. They may be required to take antibiotics regularly. There are liver transplant options available in case of acute metabolic episodes. (Philadelphia, 2022).
