The nurse’s advocacy capacity regarding changes and developments becomes more powerful through these collaborations. "Long-Term Care Resources: Essential Services from Key Agencies/Organizations" American Heart Association offers various helpful services, including health education, nutrition counseling, and chronic condition treatment of high blood pressure and the cardiovascular system. (Aspry et al., 2018) The services target those poor people who find it necessary for their welfare and future living by being provided with critical support. The Head Start Program provides low-income families comprehensive preschool education covering health, nutrition, family engagement, and development. (Gault, Cruse, Holtzman, & Contreras-Mendez, 2019) These crucial child services promote early childhood development, ensure school readiness, and offer far-reaching benefits. Head Start has significantly enhanced childhood well-being in education, health, and nutrition, laying the foundation for sustained prosperity. Community health centers are important in offering quality primary health care services to minority and poor groups, especially low-income patients. (Balcazar, Alvarado, Cantu, Pedregon, & Fulwood, 2019) They provide a vital healthcare service that meets the chronic healthcare requirements of the community; hence, patients have access to quality medical care, preventive
3 measures, and health-focused information. These centers go a long way in enhancing general health and improving the living standards of these groups . "Client/Patient Demographics and Funding Structure" This includes more than one million American families and their children with the aid of Federal grants, as well as necessary preschool-age education, health care, and nutrition programs for them. The sizeable outreach and financial support allow the program to enormously affect the child development and health status of many poor kids. This approach helps stabilize these families in the community and provides a full range of services for long-term success. Millions of patients are served annually through community health centers, an important part of the US healthcare system. (Aspry et al., 2018) They receive their money through federal grants and payments for the services they provide. Therefore, this financial support allows the centers to continue their business and extend their outreach to cater to many patients to improve public health. Enhancing Patient Care through Agency Collaboration The nurse partners with the American Heart Association by conducting community health fairs, holding lectures in various clinics and urging patients to use the organization's online resources. Patients are empowered to assume ownership of good health, which enhances preventative measures and improves general health for the broader community. Working with the Head Start Program involves screening all children by the nurse, providing their parents with health education such as nutrition and milestone development education, and referrals to Head Start for early literacy, school readiness, and other important social services. This partnership addresses the complete community health needs of children and their parents.
