2 Population Health Data Paper A. Population of interest The population of interest is Ford County, Kansas. It is ranked in the 54 th position among the 104 counties found in Kansas. Overly, this county is ranked in the lower middle range of counties in Kansas in terms of quality of life as well as length of life. In terms of health factors, it is listed among the least healthy counties found in Kansas. B. Sociodemographic Profile Sociodemographic Profile Discussion The population estimate for Ford County as of July 2021 is approximated as 33,848. The white race dominates with a 91.7% composition. This justifies the reason that English is the most spoken language in Ford County. Whites with Hispanic or Latino origin are approximated as 58.0% of the total population while those without Hispanic or Latino origin form 36.9% of the total population (United States Census Bureau, n.d.). Ford County comprises of a higher percentage of persons aged under five years. It has an 8.1% unlike U.S, which has a 5.6%. However, persons aged above 65 years take 11.9% of the total proportion. Ford County has a higher proportion of foreign-born persons amounting to 23.9% than the country’s proportion of 13.6%. Ford County has more uninsured seniors totaling to 20.0% compared to national rate of 9.3%. However, it has lower proportion of seniors with disability comprising of 7.6% of its population (United States Census Bureau, n.d.). Additionally, it has a lower percentage of high school graduates of 75.1% compared to the national rate of 88.9%. Similarly, Ford County has a lower per capita income, $26,753 compared to the national one, which amounts to
3 $37,638.However, the rates of persons in poverty, 11.6% slightly equates the national rate, 11.5%. C. Ford County Health Outcomes County Health Trends and Rankings Discussion Statistical information that highlighted the health trends and rankings for Ford County, State of Kansas and United States of America were derived from County Health Rankings and Road Maps (2022). The data gave a comparison of three entities that included Ford County, State of Kansas and United States of America. It is an actual comparison of how the smaller county faired compared to the entire nation. In some aspects, Ford County appeared healthier than the entire nation while other information still gave the benefit of doubt that Ford County would improve its health status. D. Health Factors D2. Purpose of Health Factors Data Comparison The aim of the health factors data comparison was to compare how healthy Ford County is compared to the state of Kansas and United States of America in general. It is imperative that there are aspects that Ford County fairs well than the nation as a whole (County Health Rankings & Roadmaps, n.d.). The comparison also seeks to highlight other areas that Ford County can work on to improve their overall health and ranking. D3. Health Factors Data: Successes and Challenges

4 Currently, Ford County has lower percentages of vehicle crash deaths because of excessive alcohol intake, 7%. This is lower compared to the 19% for State of Kansas and 27% nationwide. Between 2016-2019, Ford County recorded zero cases but has still seen an increase afterwards though it is still lower than the nationwide cases. The excessive drinking records a similar trend where it is given as 18% against the national rate of 19%. This is also reflected on number of sexually transmitted infections, which are still lower compared to the nationwide cases (County Health Rankings & Roadmaps, n.d.). Ford County recorded 470.0 cases in 2022 compared to 481.3 nationwide cases. This implies that a positive trend on excessive alcohol intake has also affected positively the rate of sexually transmitted infections. Additionally, Ford County has recorded a significantly lower rate of children in poverty, 14% compared to the nation at large, and 17%. This is also the same trend reflected in unemployment where Ford County recorded a 2.1% while U.S recorded a 5.4% rate. The rate of air pollution in Ford County is also slightly lower than the national rate. It recorded a 6.5 against the national average of 7.4 between 2017 and 2019. However, there are a few areas that seem challenging and Ford County would need to work on in order to improve its overall health and ranking. The rate of adult smoking in Ford County, 19% is higher than the national rate, 16%. Adult obesity rates also assume the same trend where Ford County has a 42% rate while United States has 32% rate. Citizens in Ford County have lower access to exercise opportunities, 83% compared to national access, and 84%, which may influence the adult obesity rates (County Health Rankings & Roadmaps, n.d.). Ford County has a 34% rate of physical inactivity compared to the national rate of 22%. Despite Ford being a healthy county, I believe the issue of deaths due to excessive alcohol intake have not yet being addressed. From 2019, the graph shows a sharp upward trajectory, which implies that they
5 are on the rise and there is need to have measures put in place to allow it, remain at 0% as it happened between 2016 and 2019. E. Analysis and Proposal Significant Findings From figure 9 , which depicts the trends of adult obesity rates in Ford County, Kansas and national level. Ford County seems to struggle with the higher rates of obesity. Adult obesity in Ford County has a 42% rate while United States has 32% rate according to (County Health Rankings & Roadmaps, 2020). In 2019, the adult obesity was given as 43% in Kansas, against the 36% in Kansas and 32% national rate. Looking at the trend, it is evident that Ford county had rates of adult obesity than Kansas state and national level. Citizens in Ford County have lower access to exercise opportunities, 83% compared to national access, and 84%, which may influence the adult obesity rates. Ford County has a 34% rate of physical inactivity compared to the national rate of 22%. Additionally, the rate of adult smoking in Ford County, 19% is higher than the national rate, 16%. Being physically inactive usually predispose adults to a higher risk of obesity. Additionally, having a lower access to exercise opportunities would mean that the adults would not keep fit and hence would be subjected to a higher risk of obesity. Similarly, higher smoking rates also predisposes people to obesity and other health conditions. Service or Program With the available health behaviors and statistical representation given in the trend graph in figure 9, there is need to have a program that would address the increased rate of adult obesity through fast-paced development of mobile technologies, which would help in
6 obesity prevention and treatment. The focus will be on obesity-related behaviors such as physical activity and sedentary behavior as well as physical environment and social context. A nurse would need to address such behaviors by implementing Just-in-time adaptive interventions which would be supported using wearable and deployable sensors that would be linked wirelessly to watches or smartphones would continuously track the behaviors of individuals and would in turn offer information that would assist in Just-in- time adaptive interventions (Ryan, Dockray & Linehan, 2019).The interventions would also be delivered through short message systems (SMS) where the adults would be expected to follow them appropriately. The department of health would play a key role in accessing and generating SMS in return as a shift intervention to the context information offered by Wearable and deployable sensors wirelessly linked to watches and smartphones. Action Plan The issue of increased rates of obesity in Ford county as depicted in the trend graph in figure 9, needs to be addressed. The first step will include forming a taskforce that would help in implementing the set programs. The taskforce will include some representatives from the health department and ICT department from Ford County. Additionally, it will include local health systems who would also help in rolling out the given project. A representative from the local government will be a key member in that he or she will help in bringing the proposal to life and tracking its progress. The proposal basically would entail implementing Just-in-time adaptive interventions supported using wearable and deployable sensors that would be linked wirelessly to watches or smartphones would continuously track the behaviors of individuals and would in turn offer

7 information that would assist in Just-in-time adaptive interventions (Ryan, Dockray & Linehan, 2019).The interventions would also be delivered through short message systems (SMS) where the adults would be expected to follow them appropriately. The department of health would play a key role in accessing and generating SMS in return as a shift intervention to the context information offered by Wearable and deployable sensors wirelessly linked to watches and smartphones. Raising Public Awareness and Promoting Public Engagement This is a technology-based program that would help individuals be less inactive and be able to have better access to exercise activities. Representatives of the local health systems will be the key players in this project where they will educate the public about the program and make them familiarize with the wearable and deployable sensors. These sensors would be a technology that would help individuals to make wiser as well as informed choices (Wu & Luo, 2019). A detailed information about the new mobile technology will be aired out through local Television network, Facebook, Billboards as well as through local news programs that would help the target population understand about the aim of the project and get key explanations on how the wearable and deployable sensors will be used. Lastly, a text will be send to all the target population regarding the availability of the deployable and wearable sensors. Evaluating the Service or Program The first tracking will be done through the number of individuals who acquire the wearable area networks, which are Bluetooth, enabled. This will offer a data of individuals who have access to such devices. The second tracking will be through the secured server
8 that will be receiving and analyzing the data that is transmitted from the mobile phone app. This is a system, which focuses on participants’ physical activity as well as sedentary behavior based on personalized algorithms. Another phase of tracking will be done in the local health centers or health outreaches where nurses and community health workers will inquire on whether one has the wearable health networks and issue them with some if one does not have. They will also help them download the mobile app that would contain personalized algorithms. The results of the program will be tracked for a while year and a report will be availed to determine whether it was successful or not and any challenges faced will be addressed to make it successful. This program is going to be implemented and maintained for four years.
