Description of Field Experience Activity: During this portion of my field experience on October 4, 2023, I focused on palliative and hospice care, specifically in the context of admitting a patient with advanced lung cancer to hospice care: 8:00 AM - 2:00 PM (6 hours): The day began with a 6-hour admission process for a patient who had been diagnosed with stage four lung cancer and was experiencing severe shortness of breath and pain. The activities during this time included: Admission: I admitted the patient to hospice care, recognizing the advanced stage of their disease and the need for comprehensive symptom management and support.
Collaboration: I collaborated with Durable Medical Equipment (DME) to arrange for a hospital bed, ensuring the patient's comfort and mobility. Additionally, I worked with the pharmacy to obtain a care pack of medications necessary for symptom management. Tuck-In Visit and Assessment: A tuck-in visit was conducted, which involved assessing the patient's overall well-being, addressing any immediate needs, and ensuring they were comfortable and prepared for their transition to hospice care at home. Sphere of care This field experience activity directly relates to the sphere of Palliative/Hospice Care. The nurse's role in this context is to provide comfort, symptom management, and support to patients with life-limiting illnesses, ensuring they have a dignified and peaceful end-of-life experience.
