2 WK 5 Practicum Journal Entry # 2 Nursing practice is dynamic with increase in complexity of patient issues, new diagnostic techniques and also some changes in practice. Nurses get exposed to different patients with different concerns, some of which are challenging. However, they are required to use their clinical knowledge, technical skills and their critical thinking to provide the best services. In my practicum experience, I’ve come across different patients’ cases with different cases. I admit I’ve learnt a lot, sharpening my skills throughout. This journal reflects the most challenging case scenario I found during the practice, what I learnt and how I would handle the patient different from my preceptor. It will also display the challenging cultures that I experienced when handling patients during the practicum. Case Scenario I experienced several interesting cases which I feel helped me to gain skills and confidence in providing peds care. One of the interesting cases I experienced in my clinical experience was a 16 y/o female accompanied by her mother. Her mother reported that the patient has been losing weight recently at a huge rate. She also explained that the patient has been missing some periods and was not eating. She recently noticed some laxatives in her room and was waking up at night to do exercises. She had been attending gym classes more than ever explaining to her mom that she wants to maintain her shape. The patient had However, the patient reported that she is okay with her conditions. This case was interesting since obtaining the patients information solely depended on developing rapport with the patient. I learnt some skills to use when obtaining history from adolescents especially those who are embarrassed to expose their health concerns. After empathizing with her, the patient later reported having increased stress life and was embarrassed due to her weight. She was therefore exercising, restricting food and self-inducing to vomiting.
3 What I Found Challenging I found it challenging in the assessment stage. Initially, I lacked the confidence in communicating with this patient since she didn’t want to say anything regarding her health. Without enough patient’s information, it can be hard to diagnose the patient. The patient felt embarrassed and was reluctant to explain about her present illness. I believe without the patient’s health history, there could have been possible misdiagnosis. This is because there needed information to help narrow down the etiologies, secondary and primary connections between weight loss and dysmenorrhea. In addition, it was difficult to identify whether the patient required emotional support or not. What I Would Do Differently Basically, adolescence is considered a stage where adolescents face hormonal challenges, sexual development and physical growth. According to Cassiani, et al, (2022), handling patients in this stage requires specific communication approaches and caregiving skills. If I was handling this patient, I would first initiate a welcoming space for the patient. This can be through empathizing with the patient, active listening and supporting her rather than just assuming. Despite that the information provided by her mother was enough for diagnosis, the patient still needed emotional support. I would therefore give the patient more time, interact with her and educate her other alternatives to help her manage weight rather than restricting food. Considering that she had fear of gaining weight, she would still continue with her behaviors regardless of the medication provided. Challenging Cultures There are several cultures that may influence care given and interventions that are selected for patients. The cultures may also affect the social interactions and the autonomy of the patient (Chen & Farruggia, 2018). For instance, in this case, the patient was reluctant to

4 talk in absence of her mother. She had low self-esteem which made it hard to develop interactions with non-familial others. Conclusion In conclusion, nurses come across different situations some of which are challenging. These incidences help nurses to gain confidence and improve their skills in handling more complex cases. This case highlights the importance of understanding patients’ perspectives, and approaching patients uniquely. Patients needs to be involved fully to the treatment plan, and initiate trust which in-turn helps in improving medication adherence.
