Dianne, thank you for the opportunity to read and respond to your discussion thread. You did a fantastic job explaining the financial difference between for profit and nonprofit hospital organizations. I would love to further discuss this topic below. I was particularly interested in your discussion thread because you started out mentioning HCA, which is a company I have worked for over 10 years. You mentioned how empirical data suggests that for profit organizations don’t necessarily outperform nonprofit or public organizations in terms of efficiency. Scalesse (2013) suggests that the high cost of healthcare and the uncertainty about future earnings and government reimbursements have left many nonprofit hospitals in financial positions to consider changing ownership to for-profit. Many question the consideration of change in ownership when making cuts in benefits, wages, staffing, and supplies is generation higher, revenues and CEOs are bringing home larger annual salaries. Scalesse (2013) explains that “not-for-profit hospitals are now concerned with how Medicare changes will affect their profitability and quality of care” (p. 39). Medicare Prospective Payment System payments have been reduced and hospitals now face more than $300 billion in reductions to Medicare (Scalesse, 2013). According to Scalesse (2013), the Hospital Corporation of America (HCA), which controls 163 investor- owned hospitals and has seen a profit growth in the value of its holdings to over $100 billion. This could be a reason with nonprofit hospital organizations are looking to change ownership to increase profits. According to Cronin et al. (2021), there are significant financial incentives to contribute to population health improvement because for-profit hospitals do not have the same federal tax requirements to contribute to community health and well-being. Cronin et al. (2021) suggests that for-profit hospitals seem to have the potential to serve as economic anchors in some of the same ways their nonprofit counterparts do – by providing jobs to people in the communities they serve, incentivizing their employees to live nearby and support the local economy, and through the acquisition of hospital supplies from local businesses. Cronin et al. (2021) suggests that if for-profit hospitals are able to recognize the role they play within their communities and their ability to affect the community’s well-being, economic, or otherwise, these institutions may find it mutually beneficial to themselves and their communities to leverage this potential by building stronger cross-sector partnerships. Biblical Integration For-profit organizations encourage ethical decision-making at all levels of the organization based on biblical principles of honesty, integrity, and fairness. Proverbs 11:3 states “the integrity of the upright shall guide them: but the perverseness of transgressors shall destroy them” (NIV, 2020, p. 235). This verse emphasizes the importance of integrity when it comes to business and ethical dealings. It also focuses on being honest and transparent about patient care, billing, financial management, and interactions with suppliers and partners. It provides focus on the importance of providing the highest quality of care to patients and to uphold the hospital’s mission and commitment to ethical medical practices, ensuring that patient well-being is the top priority.
