Mechanism of action: works by killing bacteria in the intestines
3 most Important Nursing Considerations and/or Adverse Effects for this drug:
a.
Monitor signs of hypersensitivity reactions and anaphylaxis, including
pulmonary symptoms (tightness in the throat and chest, wheezing, cough
dyspnea) or skin reactions (rash, pruritus, urticaria).
b.
Report signs of “red man” syndrome, including a red, maculopapular rash that
typically appears on the face, neck and upper torso, and may be accompanied
by pruritus, urticaria, erythema, angioedema, tachycardia, hypotension, and
muscle aches.
c.
Hearing loss can occur from this drug. Inform patient to tell you if they have
any ringing in the ears, dizziness, feeling of fullness in ears. Observe for signs
of loss of balance,
Important Contraindications and/or Interactions:
Adjust dose in renal failure. Ototoxicity and nephrotoxicity may occur and may be
exacerbated with concurrent aminoglycoside use.
Any special considerations for administering this drug to your client?
Use total body weight for obese patients when calculating dosages. “Red man syndrome”
associated with rapid IV infusion may occur. Infuse over 60 min (may infuse over 120
min if 60-min infusion is not tolerated). NOTE: Diphenhydramine is used to reverse red
man syndrome. Monitor injection site for pain, swelling, and irritation. Report prolonged
or excessive injection-site reactions to the physician.

Use the medicine at the same time every day. Tell the doctor immediately if the child
experiences dizziness, wheezing, shortness of breath, itching, hives, flushing of the upper
body, or muscle pain or spasm of the chest and back. Do not infuse the medication any
quicker than directed. Follow the directions/prescription exactly. Use the medicine for the
full prescribed time.
