A patient has been waking up in the middle of the night with abd pain which happened x3 this week. He was also experiencing occasional epigastric pain associated with N/V. He decided to schedule an appointment with his gastroenterologist. The doctor listened to his s/s. He noted that the patient was experiencing dyspepsia, and epigastric and supragastric pain. An endoscopy was performed at a hospital later that week. During the procedure, a long, thin tube was inserted into the oropharynx and directed into his digestive tract. The endoscope is equipped with a claw-like structure used to obtain tissue samples from the gastric mucosa. The endoscopy revealed that the patient has erosive gastritis r/t NSAID abuse. He was educated on the use of NSAIDs and was informed of the consequences including adenocarcinoma of the stomach, hematochezia, and/or hematemesis TERM/ABBREVIATION

A 76-year-old woman was seen in the GI oncology clinic for management of an adenocarcinoma. The patient presented to her PCP with x four-week hx of intermittent CP radiating to her back. An evaluation for cardiac etiology was unremarkable, including an abdominal u/s. She has epigastric and abdominal pain. An upper endoscopy and colonoscopy were obtained and showed a large neoplasm in the fundus. Pathological examination of the tumor showed adenocarcinoma. CT scans of the chest, abdomen and pelvis showed enlarged lymph nodes in the stomach within the gastrohepatic ligament. She had no nausea, vomiting, or change in BM’s. Her PmHx was notable for H. pylori
infection, DM II, HTN, CAD, GERD, and diverticulitis. There is no FmHx of stomach CA, but her older brother died of colon CA at age 55. On exam, the wt was 182 lbs, BP 182/90 mm Hg, P: 90 bmp, and the T: 97.2 degrees Fahrenheit. There was mild LUQ tenderness with a palpable mass in the same area. Results of the CBC are pending.
