AHIMA Competencies : • Validate assignment of diagnostic and procedural codes and groupings in accordance with official guidelines. 1. Refer to the guidelines for “Destruction” in the CPT Surgery Section of your CPT coding book. Per the guidelines, what is the definition of “destruction “? "Destruction" refers to the eradication of tissue by any method. This includes, but is not limited to, electrosurgery, cryosurgery, laser surgery, and chemosurgery. 2. Refer to the guidelines for “Skin Replacement Surgery” in the Surgery Section of your CPT coding book. Per the guidelines, what types of materials are classified under “Skin substitute grafts?” According to the guidelines for "Skin Replacement Surgery" in the Surgery Section of the CPT coding book, materials classified under "Skin substitute grafts" include synthetic, biological, or biosynthetic materials. These products will form a sheet scaffolding for skin growth. Skin substitute graft codes are not to be reported for application of non-graft wound dressings (e.g., gel, powder, ointment, foam, liquid) or injected skin substitutes. 3. Refer to the guidelines for “Repair (Closure)” in the Surgery Section of your CPT coding book. Per the guidelines, how would repairs/closures for wounds on the same anatomical site or the same classification be coded? Per the guidelines for "Repair (Closure)" in the Surgery Section of the CPT coding book, repairs/closures for wounds on the same anatomical

Refer to the guidelines for “Destruction” in the Integumentary System subsection in the Surgery category of your CPT coding book. List one type of lesion and one type of destruction method from guidelines. From the guidelines for "Destruction" in the Integumentary System subsection in the Surgery category of the CPT coding book, an example of a type of lesion is "benign lesions." A type of destruction method is "cryotherapy." 5. Refer to the guidelines for Repair (Closure) in your CPT coding book. Per the guidelines what types of materials are used in a wound closure? According to the guidelines for Repair (Closure) in the CPT coding book, materials used in a wound closure may include sutures, staples, tissue adhesives, and other devices. 6. Refer to code range 15773 – 15775 in your CPT coding book. Per the instructional notes, how many times can code 15773 be reported during the same session? The instructional notes for code range 15773 – 15775, code 15773 can be reported only once per session. 7. Refer to the guidelines for Breast procedures in your CPT coding book. Per the guidelines, if a puncture aspiration of a breast cyst is done, what codes would be used to report imaging guidance that is performed in conjunction with this procedure? If a puncture of a breast cyst is performed, in conjunction with imaging guidance, it would be reported using codes 19081-19086, depending on the imaging guidance modality used. 8.Refer to the guidelines for Breast repair/reconstruction. Per the guidelines, what is the goal of breast reconstruction? The goal of breast reconstruction, according to the guidelines for Breast Repair/Reconstruction, is to restore the appearance of the breast, either partially or completely, following mastectomy, injury, or deformity.

Techniques or procedures not included in codes 15570 – 15738 for Flaps (Skin and/or Deep Tissues) include adjacent tissue transfer, rotation flap, and other local flaps. 10. Refer to the guidelines for “Mastectomy Procedures” in the Integumentary System subsection in the Surgery category of your CPT coding book. Per the guidelines, what types of procedures are not included in the description for code 19303? According to the guidelines for "Mastectomy Procedures" in the Integumentary System subsection in the Surgery category, procedures involving reconstruction or revision of the breast are not included in the description for code 19303. 11. Refer to the general coding guidelines for the Musculoskeletal System. Per the guidelines, what is the definition of “Open treatment”? In the general coding guidelines for the Musculoskeletal System, "Open treatment" is defined as any procedure in which the skin or mucous membrane is incised, or a previously closed wound is opened, to allow manipulation of the bones or joints. 12. Refer to CPT code 22206. Per the instructional note for this code what code(s) cannot be reported with code 22206? Refer to the section on grafts and implants in the CPT Surgery Musculoskeletal System subsection.
