Neida Caro-Boone. The Uniform Hospital Discharge Data Set (UHDDS) is the format that was created to collect data on patients who have been discharged from hospitals. UHDDS was created to have a standard definition to provide consistent data to as many providers as possible. The Uniform Hospital Discharge Data Set also includes information about the patient’s diagnosis, treatment, and discharge status. The format created for the Uniform Hospital Discharge Data Set was created to make the patient's data easily identified, readily defined, uniformly recorded, and the patient data is easily abstracted from the medical record. The Uniform Hospital Discharge Data Set requires all Medicare and Medicaid patients’ information to be obtained in the UHDDS format for reporting but is also used by many other healthcare agencies. UHDDS demands documentation to include information such as the patient’s name, age, sex, race, ethnicity, and medical history. UHDDS expects the designation of a principal diagnosis, which is the condition majorly responsible for the patient's admission. There are several uses for UHDDS such as reporting inpatient data in acute care, short- and long-term care facilities, home health agencies, psychiatric hospitals, nursing homes, and hospices. When coding and reporting data there are always ethical guidelines that should be followed. The Uniform Hospital Discharge Data Set expects the correct designation of the principal diagnosis, The presence or absence of codes that correlate to any complications, comorbidities, or major complications as defined by the Medicare Severity Diagnosis- Related Group system the patient may have, and any procedures performed. Improper coding and over-reporting are unethical and considered fraudulent practices. The failure to properly complete all documentation, including all diagnoses and procedures can result in a monetary loss. All accurate and ethical ICD-10 coding very much depends on the coder properly following all the official guidelines and adjusting when required in your reporting because you do not want to develop improper coding practices.
