2 Surgery Guidelines: CPT Codes 40490 - 60699 System: Urinary Title: Urinary System – Kidney – Excision – Renal Transplantation Guidelines: Renal autotransplantation involves the reinsertion of the patient's own kidney as the main procedure, followed by additional procedures outside the body (such as partial nephrectomy or nephrolithotomy), which are indicated by modifier 51 (refer to code 50380 and relevant secondary procedures). On the other hand, renal allotransplantation comprises three distinct aspects of the physician's work: Donor Nephrectomy: This can be from a deceased donor (cadaver) or a living donor. It includes obtaining the graft(s), preserving them in a cold storage solution, and caring for the living donor (code 50300 for cadaver donor nephrectomy and 50320 for living donor nephrectomy). Backbench Work: This involves preparing the renal allograft before transplantation. For cadaver donors, it includes tasks like removing fat, dissecting attachments, excising the adrenal gland, and preparing the ureters, renal vein(s), and renal artery(s), with ligation of branches as needed (code 50323). In the case of living donors, similar preparation is done, either through open or laparoscopic surgery, including fat removal and the preparation of ureter(s), renal vein(s), and renal artery(s), also with ligation as necessary (code 50325). Recipient Renal Allotransplantation: This final phase encompasses the actual transplantation of the allograft into the recipient, with or without the removal of the recipient's own kidney, along with post-operative care for the recipient (codes 50360 and 50365). These codes are used to describe the various components and steps involved in renal transplantation procedures. Using the Medicare Physician Fee Schedule (MPFS) look-up tool, here are five CPT codes related to the Urinary System and their respective postoperative periods: 1.) 50320 – open, from living donor – 90-day Postoperative Period 2.) 50360 – Rental Allotransplantation, implantation of graft; without recipient nephrectomy – 90-day Postoperative Period 3.) 50365 – With recipient nephrectomy – 90 Day Postoperative Period 4.) 50370 – Removal of transplanted renal allograft – 90 Day Postoperative Period 5.) 50380 – Rental Autotransplantation, reimplantation of kidney – 90-day Postoperative Period
