1. What were the probable causes that led to the fatal accident? The Nation Transportation Safety Board determined the cause of crash for Flight 191 was an asymmetrical stall that became a roll(spin). Wind shear caused the left wing to stall due to a thunderstorm applying a down draft. I would like to contact the FAA and inform them that I recommend that when checking/ monitoring the weather from the tower to take an alternative route and not worry about the cost in fuel as the cost of lines is higher than jet fuel. First and foremost the Pilot in command is held the most responsible. He is in charge and is supposed to make the best decision for the aircraft and the safety of those aboard. The ATC and flight crew may play a role in not having a more extensive protocol. The FAA is in charge of creating a safe environment in the aviation industry. It is their purpose to set protocols in place to ensure everyone flying is doing so in a safe manner.
