Answer 1: A. The Unsafe Acts: Trying to modify the machine without turning off the power was one of the risky behaviors. Using both hands to reach into the machine while it was still running. Using her leg to unintentionally press the one-button actuating control. B. The Unsafe Conditions: Among the dangerous situations were ones where the material was not properly aligned with the stencil and the holding fixture's securing device was broken. C. The Energies Involved: The worker's body was too close to the one-button actuation control, making it easily activated by accident. The worker could not have been injured by the fixture clamps or feed mechanism on the machine because it required sufficient protection or sensors. D. The steps to be taken to prevent this situation from Recurring: Machine guarding improvements like interlocking guards will limit access to dangerous areas. To reduce unintentional activations, control buttons should be moved, and a two- handed control mechanism put in place. Apply Engineering control, risk control and do an audit of the high-risk injuries area regularly. It is essential to have thorough training on correct protocols and the significance of turning off the power before making any alterations. All these actions will make the workplace safer. Answer 2: It is my recommendation to put in place a thorough safety management system for the plant's hazardous equipment, such as the zinc plating machine. A safety management system is an arrangement of guidelines, protocols, and practices designed to stop or lessen the possibility of accidents, diseases, and property damage resulting from work-related activities. Several essential components of a safety management system include: A. Employee involvement and consultation:
Employees should be involved in the detection, evaluation, and mitigation of dangers as well as the assessment and enhancement of safety performance. Identification and assessment of hazards and risks: It is important to actively identify and evaluate the risks and hazards related to the plating machine and other equipment, and to document and share the findings. B. Control and elimination of hazards: The hierarchy of controls, which gives priority to engineering, administrative, and personal protective measures, should be used to remove or reduce the risks and hazards. C. Training and instruction: The employees need to receive instruction on how to safely operate, maintain, and repair the plating machine and other equipment in addition to learning about emergency protocols and first aid techniques.
