Peer Feedback Two 1d. Written Reflection During this phase, I created a project report for my BBFE HIP. This was not something I have done before so I took a moment to google some examples of what should be included, and closely read the rubric to ensure everything was included in the project report they were asking for. In the report, I included a brief background about the project, the project's goal, data elements, target metrics and who was responsible for collecting and presenting the data. There are several data elements I chose to include, many of them are already collected by Employee Health nurses and kept in our data management files. I also chose to include some that were not currently being tracked so I could better evaluate the success of the HIP such as the Date of BBFE education, If errors occurred in the process and what error was made, and Project survey results from surveys given pre-procedure change and post-procedure change, given electronically. The following metrics were identified to also help with the evaluation of the BBFE process changes, Employee satisfaction score, Education satisfaction score, Follow-up care/treatment received, and Ease of accessibility and clarity of BBFE information. The responsible party for data collection and presentation was of course myself as the project manager. I will put everything into a spreadsheet for review. Once data is collected it will be placed on a bar graph, comparing pre-implementation data and post-implementation data and presented to the Employee Health management and team during the monthly “All Hands” meeting. Each metric will be addressed during the meeting and suggestions will be made by all Employee Health team members. D159 Evidence-Based Measures for Evaluating HIP Page 3

Peer Feedback Two 2c. Written Reflection During this phase I created a data management plan for the BBFE HIP that I will be implementing. Again this is something I've never done before but I felt like they laid everything out really well with this task as to what is expected. My data will come from reviewing charts in OHM (the occupational health management program we use) as well as the exposure data management excel sheet that we utilize, and pre/post implementation surveys. The measures I identified were employee satisfaction score, education satisfaction score, follow-up care/treatment received, ease of accessibility and clarity of BBFE information, and occurrence of errors. I also identified two key performance indicators and had to create my own benchmark for expectations. They included Decrease in BBFE process errors 50% fewer errors made as the benchmark and Increased employee satisfaction scores 85% satisfaction score or higher as the benchmark. I also identified specific parameters for data collection and the process that will be used to analyze the data. D159 Evidence-Based Measures for Evaluating HIP Page 6
