What do I need to do to achieve a satisfactory result?
To achieve a satisfactory result for this assessment all questions must be answered correctly and all parts of the observable components must be performed to a satisfactory level as indicated in the criteria section of the Observation Checklists.
If your assessment is not satisfactory you will be provided with feedback and asked to resubmit responses to those questions you
answered incorrectly within two weeks, unless otherwise agreed with the Head Teacher where there are extenuating circumstances.

In accordance with the TAFE NSW policy Manage Assessment Appeals, all students have the right to appeal an assessment decision in relation to how the assessment was conducted and the outcome of the assessment. Appeals must be lodged within 14 working days
of the formal notification of the result of the assessment. If you would like to request a review of your results or if you have any concerns about your results, contact your Teacher/Assessor or Head Teacher. If they are unavailable, contact the Student Administration Officer.

Contact your Head Teacher for the assessment appeals procedures at your college/campus.
Task instructions
You are required to provide evidence of how you have participated in safe work practices to ensure the health and safety of themselves and others. To do this you must complete and submit:
1.
One workplace incident report based on either a real incident or the scenario provided.
2.
One workplace risk assessment using the templates provided.
3.
Responses to short answer questions.
If your responses are based on a TAFE-approved work placement you must sign the declaration at the end of the assessment and ask a workplace representative to sign it to verify the authenticity of your assessment.
Your assessor will mark your performance using the Assessment Checklists provided. You should refer to the Checklists to understand what you need to demonstrate. All the task assessment criteria must be met. In the Assessor Comments, your assessor will describe how
you have demonstrated the required skills and knowledge.

Part 1: Workplace incident report
You are required to complete a workplace incident report using the template provided below. If you undertake a TAFE-approved work placement, this should be based on a real incident if one occurred. You must maintain each person’s privacy and confidentiality by not using their real names or other identifying details. If no incident occurs, or if you do not undertake a work placement, you must instead complete the incident form using the scenario provided below.

Today, Ian White, a visitor to your workplace, tripped on a waste bin in the foyer at midday. He has a cut on his left elbow, which is bleeding and is complaining of a sore right knee. His date of birth is 01/01/1999 and his contact details are 23 Rylands Dr, Orange NSW 2800, phone 02 6565 0011.
The receptionist, Piper Holdings, witnessed the event and attended first aid. Her contact details are 5/7 Honsy Dr, Bathurst NSW 2795, phone 02 7766 5543. Next of kin did not need to be contacted.

Part 2: Workplace risk assessment
You are required to provide evidence of how you have conducted a workplace risk assessment and recorded the results, either while on work placement or in a simulated workplace at TAFE NSW. Refer to their class Teams site for further information about how the assessment is to be conducted. There are 2 components to this task:
1.
Using the template provided on the following pages, conduct a workplace hazard identification and risk assessment and identify and record at least one existing hazard and at least one potential hazard.
2.
Choose one existing hazard and one potential hazard that you identified in your hazard identification and risk assessment, and complete the risk assessment template provided,
identifying:
a)
existing risk control measures
b)
current level of risk using the risk assessment matrix shown below
c)
at least one additional risk control measure using the hierarchy of control
d)
residual level of risk once the additional risk control measure has been applied
e)
who you reported this risk to, and when.
