Week 1 – Initial Discussion Post
Case management is a process used to assess clients, complete their treatment and all additional plans, implement, coordinate, as well as to monitor and evaluate all possible options, services and resources required to better help and meet the client’s needs. By assessing the client first, it can help the case manager or social workers to figure out which resources and services are needed in order to fulfill what’s best for the client moving forward. Case management is used throughout many agencies to help their clients like housing agencies, reentry agencies, mental health agencies and many more. This process serves clients dealing with mental health, drug and alcohol addictions, clients who are reentering into society, families, children and so many others.
One of the goals and functions of case management is to provide advocacy for their clients as well as their support system. Additionally, the most important goal is to achieve the best and quality outcomes for each client by providing the appropriate resources and empower the client in the best way possible.
