the specified employee to give him/her/them bad news.

Scenario
You are employed as a Benefits Analyst with Confederation Inc., and you are responsible for
collecting and submitting all required documents from employees to LifeSun Insurance
company.
All employees of Confederation Inc. are automatically enrolled in the “Standard
Health Care & Disability Insurance” when they are hired on.
Employees have a 30 day-limited
time offer upon hiring to enrol in the “Extended Health Care & Disability Family Insurance
Program,” but they must submit additional documents to qualify.
Alison Blair, an employee who desired to enrol in the “Extended Health Care & Disability
Insurance Program,” is now past the 30-day time limit to provide supporting documentation to
LifeSun Insurance.
You must email Alison (

) and tell them why their application
was denied and that you cannot grant them an extension on the 30-day enrollment deadline. Your

Upon careful review of the submitted documentation, the following documents are missing:

Certified Letter from employee’s General Practitioner:
The received letter indicates that
“Alison is not currently sick” but it lacks the required confirmation regarding you and your
family never being tested for or diagnosed with specific medical conditions (Multiple Sclerosis,
Breast Cancer, Parkinson’s Disease, or Lymphoma).

Notarized copy of Permanent Resident [PR] card (front and back):
The photocopy of front of
the PR card received, but still require the notarized copy of front and back.
4.
Notarized copy of your 2021 Canadian Notice of Assessment:
Not received.
As per LifeSun Insurance company policy there is a timeframe of 30-days, and we are unable to
extend this deadline. Due to missing or invalid documentation, unfortunately, we cannot approve
your application for the Extended Health Care & Disability Family Insurance Program at this time.
