STEMing UP Program Parent Release/Media Form: I give full permission for my child, ___________________, to participate in the STEMing UP program run by hEr VOLUTION, a not for profit dedicated to promoting STEM education for young women. The purpose of the STEMing UP program is to provide girls and gender non-conforming youth with the opportunity to be introduced to a wide range of skills and experiences designed to help them achieve their life and career goals. The program can last for over two years giving participants the opportunity to learn various coding languages offered in the program. (Participants will need to be approved prior to entering the second year of the program). The duration of the program will allow participants to build long-term relationships with their peers and STEM mentors while seeing their ideas come into fruition strengthening their confidence and skills. Through this program, your child will receive theory, hands-on and experiential learning facilitated by industry professionals and peers to benefit their future career in a financially rewarding technology industry. During the 2023 program year, the program will be held from July 31, 2023 until August 25, 2023 To ensure we are accessible to participants from across Ontario, all sessions will be virtual . If an applicant requires access to a laptop during the program, a laptop may be borrowed from hEr VOLUTION (conditions apply). A signed borrower agreement will be required. Participants are allowed to keep their laptop until the end of the program and may use it for school purposes. ● I understand that full or partial scholarships are offered only to applicants who demonstrate financial need (ie. low income families/households) by submitting all required documents with application. I understand that the program fee is $3,000 and, if I refuse to submit the necessary documents to demonstrate financial need, I agree to pay the full amount before the program starts or the available seat for my child will be offered to the next applicant. ● I understand that if my child will be eligible for a Wi-Fi subsidy, that will be paid to my family at the end of the program. ● For more information, please send us an email at stemingup@hervolution.org ● I hereby give hEr VOLUTION and all its STEMing UP partners permission to use my child’s photograph publicly as a program participant.
