1. Identify the difference between assigning a number to a verbal description such as for a zip code and the activity of assigning a diagnosis or procedure code to medical documentation. Coding is the transformation of verbal descriptions into numbers provided in a classification system. Whereas assigning a zip code is a rather simple activity, the assignment of diagnostic and procedural codes requires a detailed thought process that is supported by a thorough knowledge of medical terminology, anatomy, and pathophysiology. 2. How often must the ICD-10-CM and ICD-10-PCS code books be updated and when do the updates go into effect? Official addendum documents the changes which are effective April 1 and October 1 of each year. International ICD updates occur less, and ICD-10-CM and ICD-10-PCS undergo annually in the US to remain current. 3. When were the London Bills of Mortality developed? By Whom? Why is this important in relation to ICD-10-CM/PCS? London Bills of Mortality were developed on December 27th, 1664, by John Graunt. The London Bills of Mortality was statistical data gathered through a system and arranged into numerical codes, these codes were used to measure the most frequent causes of death. In 1937, the statistical analysis of the causes of death were organized into the International List of Causes of Death. Later the list developed into the International Classification of Diseases, which is now the 10th edition (ICD-10-CM/PCS). 4. Between 1838 and 1893, what other early classification systems were developed? Bertillon Classification of Causes of Death which is a system that classifies deaths.
5. What did the American Public Health Association recommend to countries to do in 1898, and what occurred in 1900 as a result of the recommendation? That the registrars of Mexico, Canada, and the United States to also adopt Bertillon’s system and revise the system every ten years. 6. How did the World Health Organization become involved with the classification system in 1948? They took on responsibility, preparing and publishing the revisions of ICD every ten years. 7. What organization released the 11th version of the International Classification of Diseases? What are the main benefits that differentiate it from the current edition? The World Health Organization (WHO) released the 11th version of the ICD-11. In ICD-10 the different personality disorders were replaced to single personality disorders in ICD-11 which is characterized by problems in functioning of aspects of the self and/or interpersonal dysfunction. 8. How do the Electronic Transaction and Coding Standards correlate to coding and why are they important? ICD-10-CM and ICD-10-PCS are named required components of the ICD-10-CM and ICD-10-PCS code set in the modified final rule for electronic transactions and coding standards. Having this made the adherence to the guidelines a requirement for compliance with the rule. 9. What is the main difference between the ICD-10 annual revision released by the WHO and the ICD-10-CM and ICD-10-PCS annual revision released by CMS and NCHS? Identify which addenda CMS publishes and which addenda NCHS publishes along with the overall connection to the WHO. 
The World Health Organization (WHO) publishes an international revision of the code system every ten years, and within the U.S. they clinically modify the codet set annually. National Center for Health Statistics (NCHS) publish an annual addendum with the approval of the WHO, they are responsible for maintaining the diagnosis classification ICD-10-CM, and CMS is responsible for maintaining the procedure classification for ICD-10-PCS. 10. What is the importance of having the Official Guidelines as a required component of the ICD- 10-CM and ICD-10-PCS code set in the final rule of the electronic transactions and coding standards? Having the official guidelines for coding and reporting named are a required component for electronic transactions and coding standards makes adherence to the ICD-10 CM/PCS guidelines a requirement for compliance with the rule. 11. What is the purpose of the Official Addendum for ICD-10-CM and ICD-10-PCS? Documenting changes with annual updates in the U.S. to remain current (codes need to be added, revised and/or deleted) and may be found at the NCHS website. 12. What government agencies provide the representatives to chair the ICD-10-CM Coordination and Maintenance Committee, and what is the committee’s responsibility? The ICD-10 Coordination and Maintenance Committee is a Federal interdepartmental committee comprised of representatives from Centers for Medicare and Medicaid Services (CMS), Centers for Disease Control and Prevention’s (CDC) and National Center for Health Statistics (NCHS).
