Local Anesthesia Dose Calculations Homework/Practice Problems
Name: _____________________________________________ Date: _____________________
1. What is the total amount of Lidocaine contained in three cartridges of 2% Lidocaine w/ 1:100,000 epi?
2% = 20 times 1.8ml = 35 ml per cart. 36mg per cart times 3 carts =
108 mg 2. Calculate the max. dose a healthy 125 lb adult can receive for the following anesthetics:
a. 4% Septocaine w/ 1:200,000 epi
articaine MRD 3.2 mg times 125 lbs = 400 mg MRD articaine
4% = 40 times 1.8ml =72 ml of art per cart
1/200 times 1.8ml = 0.009 ml epi per cart MRD 400 divided by amount in each cart (72) = 5.5 carts Answer: 400 mg or 5.5 carts
b. 3% Mepivicaine plain
MRD mepivacaine 3.0 times 125 lbs = 375 mg Max 30 times 1.8 = 54 mg per cart 375 divided by 54 = 6.5 carts
Answer: 375 ml or 6.5 carts
c. 4% Citanest w/ 1:200,000 epi
prilocaine MRD 4.0 times 125 lbs = 500mg MRD
40 times 1.8 ml = 72 mg per cart 500 divided by 72 = 6.5 carts 1
3. Calculate the max. dose a 75 lb child can receive for the following anesthetics:
a. 4% Septocaine w/ 1:200,000 epi
articaine MRD for child is weight divided by 150 75 divided by 150 = 0.5 mg
Then find adult MRD 3.2 times 150 lbs = 480 480 times 0.5 = 240 mg MRD for child
40 times 1.8 ml= 72 ml per cart
240 mg divided by 72 ml per cart = 3 carts
b. 3% Mepivicaine plain
MRD child mepivi ; weight divided by 150
75 divided by 150 = 0.5 mg Adult MRD 400 mg times 0.5 mg = 200 mg MRD for child
30 times 1.8= 54 mg per cart
200 divided by 54 = 3.5 carts
c. 4% Citanest w/ 1:200,000 epi
child’s weight divided by 150 (75 divided by 150 = 0.5mg)
adult MRD 600 mg times 0.5 mg = 300 mg MRD child
40 times 1.8ml = 72 ml per cart carts
300 divided by 72 = 4 carts

4. What is the max. dose a 125 lb adult can receive for the following anesthetics:
a
. 4% Septocaine w/ 1:200,000 epi (articaine) articaine 3.2 times 125 = 400 mg MRD
40 times 1.8ml = 72 mg per cart 400 divided by 72 = 5.5 carts b.
3% Mepivicaine plain
MRD under 150lbs: 3.0 times 125 = 375 MRD
30 times 1.8ml = 54 mg per cart 375 divided by 54 = 6.5 carts c. 4% Citanest w/ 1:200,000 epi (prilocaine)
MRD prilocaine under 150 lbs: 4.0 times 125= 500 MRD
40 times 1.8ml = 72 mg per cart
500 divided by 72 = 6.5 carts 5. You have a 150 lb adult patient at 8 am for a scaling and root planning procedure. At this appointment you administer 4 cartridges of 4% Articaine w/ 1:100,000 epi. Your patient continues to have sensitivity on the maxillary anteriors. How many more cartridges of this anesthetic can you administer?
MRD articaine 3.2 times 150lbs = 480 mg MRD
40 times 1.8ml = 72 mg per cart 72 times 4 carts = 288 mg already administered MRD 480- 288 administered = 192 ml to go
192mg divided by 72mg in each cart =
2.5 more carts

6. Your 180 lb patient is an ASA III due to CVD. What is the maximum number of cartridges of 2% Lidocaine w/1:100,000 epi he can have? Cardiac MRD epi = 0.04
MRD lido over 150 lbs = 500mg 1/100 times 1.8ml= 0.018 mg of epi per cart 0.04 MRD divided by 0.018 mg per cart = 2 carts (based on epi only)
LIDO:
20 times 1.8ml = 36 mg per cart
500 MRD divided by 36 = 13 carts (but we go off epi in this case, whatever MRD is lower)
7. Your 105 lb. healthy patient has received 3 cartridges of 4% Prilocaine plain. How many additional cartridges of 2% Lidocaine w/1:100,000 epi can he have?
Prilocaine MRD- 4.0 times 105lbs = 420 MRD
Lidocaine MRD -3.2 times 105lbs = 336 MRD
40 times 1.8ml= 72, 72 times 3 carts= 216 mg administered of the 336 MRD
336 – 216= 120 left
1.8 times 20= 36
120 divided by 36= 3 carts
8. The dentist you work with administered 4 cartridges of 2% Lidocaine w/1:100,000 epi to a 45 pound child. Is this a safe amount? Why or why not?
MRD lido for child of 45 lbs: 45 lbs divided by 150= 0.3
Adult MRD lido 500mg times 0.3mg = 150 mg MRD for this child
20 times 1.8ml= 36 mg per cart 4 carts times 36 mg each= 144 mg administered So, yes SAFE amount was given, 144 is under the 150mg MRD for this child

9. A dentist administers 3 cartridges of 2% Lidocaine w/1:50,000 epi to your 170 lb healthy patient. The patient is still not numb, so he then administers an additional 4 cartridges of 4% Articaine w/1:100,000 epi
. Is this a safe amount? Why or why not?
MRD lidocaine over 150lbs is 500mg (lowest MRD)
MRD of epi is 0.2mg MRD of articaine is 3.2 times 170lbs = 544 mg MRD
Lido:
20 times 1.8ml = 36 mg per cart 3 carts given times 36 each= 108mg of lido given
Epi:
1/50 times 1.8ml = 0.036 3 carts given times 0.036 each = 0.108mg epi given Articaine 4%: 40 times 1.8ml = 72 mg per cart, 4 carts times 72mg each = 288mg of articaine Now take MRD of 500mg minus 108 mg (lido) minus 288 mg (articaine)= 104mg Of the 500 MRD there was 104mg to spare far as LA goes
Articaine epi: 1/100 times 1.8ml = 0.018 mg 0.018 per cart times 4 carts = 0.072 mg of epi given.
Now MRD 0.2 minus 0.108 (epi from lido carts) minus 0.072 (epi from articaine carts) = 0.02 to spare. So, yes, safe dose. 5
