2 Advancement of the Health Care System in the United States The health care system of the United States has experienced scrutiny from all around the world and has been a debate over the years. Quality care is a frequent topic on the agenda and sometimes is motivated by reasons such as providing the public with high quality care or identifying problems specifically to healthcare quality (Busse et al., 2019). This paper will discuss how the United States has advanced its health care system by identifying what is quality care and how it has changed since the historical days of “the doctor knows best”. Providing quality care at low risks using current health care priorities will be identified and we will assess the implicit and explicit criteria on health care quality. Dimensions of Quality Health Care Though quality care is deemed important and everyone including the public is aware that high quality health care should be delivered through the health care system, many still may not understand the true meaning of quality. Definitions of quality health care were mainly developed solely by healthcare professionals and researchers (Busse et al., 2019). According to the Agency for Healthcare Research and Quality (n.d), the institute of medicine defines health care quality as desired outcomes being consistent with current and up to date professional knowledge. The World Health Organization (n.d) adds to that definition by stating that quality care is evidenced based and critical for obtaining health coverage that is universal. Young et al., (2018) explained that for decades quality was immeasurable and compared to beauty – in the eye of the beholder. Regulatory agencies focused on identifying poor quality care rather than setting standards to ensure high quality health care was being delivered. In order for the health care system to provide quality care, measuring or standardizing quality is impeccable. Therefore the dimensions of quality care must be implemented to
3 achieve a health care system that delivers quality care. The Agency for Healthcare Research and Quality (n.d) identifies six domains relating to quality care. Efficiency is one such domain that relates to utilizing the resources available to provide the maximum quality care delivered. Effectiveness and equity together refers to evidenced based care provided to achieve successful outcomes equally regardless of difference in personal characteristics or clinical conditions. Patient centeredness and safety are two other domains, which refer to meeting the patients’ needs by providing care, education and support without actual or potential bodily harm. The sixth domain is timeliness which is related to providing efficient care while limiting delays. The provision of quality services requires good governance; a skilled and competent health workforce that is supported and motivated; financing mechanisms that enable and encourage quality care; information systems that continuously monitor and learn to drive better care; medicines, devices and technologies that are available, safe and appropriately regulated; and accessible and well-equipped healthcare facilities
