Advocacy Action Plan D025 After reviewing the County Health Rankings and Roadmap website, it has become clear that Jasper County, Texas does not have to access to exercise opportunities that the rest of Texas or the United States has. A mere 34% of Jasper County has access to exercise opportunities compared to 82% of the rest of Texas and 84% of the United States. This means only 34% of the population can easily go somewhere where they can participate in physical activity. Jasper County is ranked 230 out 244 as one of the least healthy counties in Texas. (County Health Rankings & Roadmaps , 2023) This contributes to some other health disparities in Jasper County such as a high obesity rate and high diabetes rate compared to the rest of Texas as well as the United States. ( How healthy is Jasper County, Texas? 2023) Jasper County is a rural area with a poverty level of 20.3% completed to the national average of 13.6. ( How healthy is Jasper County, Texas? 2023) These populations are most likely to not be able to access gyms or structured physical activity due to lack of transportation and funds to pay for memberships. The SDOH Neighborhood and Bulit Environment addresses the issue of access to reduce health risks and promote overall health. I was not able to find any current policy within the county to address this determinant of health. My advocacy action plan includes implementing strategies to provide access to physical
activities for the community of Jasper. Some examples include, building bike and walking paths, creating activity programs for older adults, and establishing community exercise programs such as CrossFit, Zumba, or boxing. (Community Fitness Programs, 2023) Since I work at the local hospital, I have some contacts on the hospital board. I would request a meeting with the board to discuss the possibility of finding some buildings in the community that could be turned into fitness centers or community centers. We would approach the chamber of commerce and look for people in the area who be interested in being involved in creating new businesses. If unable to find buildings to purchase, we could approach local churches about letting the community use their building for exercise classes. Once these measures have been implemented, we would continue to monitor and evaluate to determine the effectiveness of the interventions.

Smart Goal The team will establish ways for the people in the community of Jasper County, Texas to access locations for structured physical activities. S (specific) Creating spaces in the community like fitness centers and walking and biking paths that are affordable and accessible. M (measurable) Follow trends on websites and monitoring systems like the county health rankings. A (achievable) Developing community interest and reaching out for volunteers to donate space for exercise classes or people to teach classes. R (relevant) Having access to physical exercise classes can increase the overall health of the community. Exercising decreases the risk of heart problems, diabetes, and obesity. T (time) This is a long-term goal that would take several months to a year to complete to either find buildings or collaborate with the local churches to find locations for exercise classes.
