Case #1: Alabama Water Crisis: What are your thoughts? The article is a very heartbreaking story to read, especially to realize how people are living in certain conditions. In the state of Alabama, there is a massive sewage problem that has been going on for many years. Public authorities have tried to resolve this issue and President Biden has spoken out about this issue many times however, no action has been taken. It is very upsetting to see the unsafe living conditions that people are living in. How does Social determinants of Health and history play a part in the area of Alabama discussed in the article? Social determinants of health and history play a very big part in this area of Alabama. This particular area in Alabama has been suffering a civil rights battle for more than 60 years now as this used to be the Black belt in Alabama where African Americans are one third of the population. The sewage problem , goes to demonstrate that their has been an extreme lack of effort in fixing this problem over the course of many years due to poverty, poor sanitation and economic disparities. Solutions are noted in the article, do you agree / disagree with the federal response to the water crisis? I can agree with the solutions that are mentioned in the article opener, I disagree with the fact that no action is being done. President Biden has addressed the concerns and voiced his plan to propose cleaner and safer conditions for people who have been affected from this, however no changes have been made. Additionally the $1 trillion dollar infrastructure act is mentioned in the article however, is this really a solution for the people health directory related to this problem. The infrastructure act more so explains the née for better health care access to the community rather than fixing the actual problem. These residents are exposed to E coli and other microorganisms or parasites, if you were an NP practicing in the area, what would you advise your patients who are affected by this water issue? There is no doubt a high and significant amount of exposed to E.coli and many other microorganisms that can negatively affect the people of this community. If I were to be treating patients in this area, I would advise all residents to boil their water as much as possible prior to drinking, avoid tap water and of course educate them about the risk of the unclean water. Additionally, I would advise stool tests frequently and annual check ups to make sure that patients of the community are getting treated for anything they may acquire. References: Thrush, G. (2022). My initial reaction and response to the ACEs was very saddening to see and frightening for all children all around to go through any traumatic experience. This can really impact a child's growth, development and well being. There is a very big correlation between adverse childhood experiences and health conditions including mental health. Many children can grow to develop anxiety, depression, bipolar disorder any many other conditions from experiencing trauma. What are the list of ACEs on the screening tool that Dr. Burke discusses during her TED talk? In Dr. Burke Harris's TED talk, she discusses categories such as neglect, substance abuse, divorce , domestic violence both experiencing and witnessing and mental illness in the household. Do you feel that children are overdiagnosed with ADHD? Why or why not? I personally do believe that many children are over diagnosed with ADHD. This is a condition that should be taken very seriously, yet each child should be evaluated efficiently, and not just slapped on with a diagnosis of ADHD and given medications to control it. ADHD diagnosis can be a result of ACEs as well and sometimes diagnosing a child with ADHD is not fixing the problem, just sweeping it under the rug. How are social determinants of health related to childhood adverse experiences? Some social determinants of health, including access to health care, and socioeconomic status can widely play a role in childhood and the adverse experiences. At such a young age, children should have to worry about if they will have a meal at night, if they are in danger and if they are a bother to their parentrs.This really molds a child into thinking they are the problem leading to ACEs. Social determinants contribute to a load of consequences and sometimes adult problems should not be inflicted on a child. What services or resources available to patients and their families that will mitigate or prevent ACEs? Some services that can be offered and are available to patient and their families to help prevent ACEs include parenting education, mental health services, especially for the pediatric population as these can be very difficult to find. Also, accessible health care services and school
based programs that offer children support. What is your role as a future NP in addressing ACEs to improve the health and wellbeing of your patients? As a future Nurse Practitioner, my role in helping to address ACEs is to educate all my patients if they are parents on the importance of preventing ACEs and using screening tools to assess if a child may be at risk for this. It is very important to try to advocate for all patients and children that might be experiencing this and find community based resources that may assist in this.
