2 D222: Comprehensive Health Assessment D1. Health Promotion Justification The two health promotions that were identified during the patient’s health assessment were, lose weight and quit smoking. Pt reported during health assessment that her body weight was 226 and her height was 5ft 10in. This places her in obese category with BMI of 32.4. Pt reported not following any diet and not exercising. Pt educated on health benefits of 5- 10 % weight loss, which include decreased risk factors for cardiovascular problems, diabetes, cancer, and improvement in knee osteoarthritis, sleep apnea, fertility-related end points, and health related quality of life. Pt educated on different ways to available to lose weight such as diet, pharmacotherapy, and surgery (Tahrani 2022). The second health promotion identified during the assessment was for the pt to quit smoking. Pt reports smoking ½ pack of cigarettes per for the past year. Pt educated on importance of smoking cessation. Education included increased possibility of acute and chronic cardiovascular problems, such as: heart attack, stroke, high blood pressure, diabetes, and more. Pt educated that after quitting harmful cardiovascular effects are relatively quickly reversed. Pt educated on different modalities for cessation, such as: counseling, pharmacotherapy, electronic nicotine delivery systems (ENDS) (Benowitz 2022).
3 D2. SMART Goals Goal 1. Pt will begin dieting and talk with her doctor about use of pharmacotherapy to achieve 5-10% (11.3-22.6lb) weight loss in the next 2 months. Goal 2. Pt will immediately begin smoking cessation via counseling, and or ENDS and discussion the use of pharmacotherapy with provider and quit smoking immediately.
