1. What types of research techniques can be conducted to better understand why the
history and physical reports were missing? Do you think a standard tool to measure
all canceled surgeries should be created?
A short, simple survey directed at patients can be one of the techniques used to understand why patients are canceling. Another would be to have reminders to update patient records to see if the
patient even goes to that facility. It would be helpful if a standard tool was created to help measure all canceled appointments. That way, there would be physical evidence as to the reason why, and from there, some form of action could be created.

Do you think the members of the meeting addressed the issue at hand? Can you think of a way the team could more effectively review solutions?
They did and did not. It was a great idea to form a subcommittee to find out the trends of what is happening in the HIM department and the OR nurse department, but they still ended up playing the blame game: OR blamed HIM while the doctor wanted faster results.
3. As a multi-disciplinary team, it is important that key personnel can provide input. Were the correct leaders invited to the discussion?
The correct leaders were invited to the discussion for the focus topic, which was based on missing reports in the OR department about patients who ended up canceling their scheduled surgeries.
4. This problem impacts multiple stakeholders. Which stakeholder do you feel is impacted the most, and why?
The HIM department is in charge of updating records that come from the various departments. The OR department would also be impacted the most, for they are losing patients to cancellations, which in turn burdens the doctors who were scheduled for that particular surgery.
