Assignment: Using the Wheel of Wellness
After reading the articles for this week and the power point, you should have a basic idea of how the Wheel of Wellness is designed and can be used in an Assessment. Your task is to utilize this procedure in a self-assessment. Note that this is for instructional purposes only and that you are not required to self-disclose any material that you are uncomfortable sharing.
You will be evaluated on your ability to apply this material; therefore, it is essential that you use the key points in the readings and power points
as you follow these procedures. Step 1: Make sure you have a good understanding of wellness, recovery and how the model works. It may help to have a picture of the model in front of you. Notice how all of the parts are connected. Your self-assessment today will be a snapshot of how you view your own wellness, which will be helpful as you consider your strengths and areas of improvement.
Step 2: On a piece of paper-
a)
Based on your readings, write down each of the five life tasks and rate your level of wellness from 1 (very low wellness) to 10 (very high wellness).
b)
Then rate your satisfaction with each of these levels on a scale from 1 (very low satisfaction) to 10 (very high satisfaction).
c)
Write a small paragraph to describe your ratings. Use the Ratings Form provided to identify and describe your ratings. (Do not forget to delete my example for Task 1.)
d)
Repeat this same process for each of the 12 Subtasks of Self-direction.
Step 3: On a separate page (1 page max), reflect on themes and patterns that emerge during your descriptions on Step 1. You do not need to have history with substance abuse to do this. You should be able to see how certain tasks/subtasks are related to one another and even identify why these themes and patterns have emerged. Step 4: On a separate page (1 page max), identify at least one life task or subtask that you would like strengthen in order to improve your personal wellness. Reflect on the wellness rating that you gave that category and name the rating you would like to see. Describe what this new level of wellness in this category would look like. How would you know that you have reached this level? What would you see in yourself that we different? How would you feel? How would you look? How would other people describe you differently? What positive traits would they see more fully?
Step 5: Develop a Wellness Plan (1 page max). Name at least three strengths that exist among your other life tasks/subtasks that you may turn to in order to reach the goal you established in Step 4. (e.g. friendship may help you exercise). Describe how you will use these strengths. Name at least three challenges that exist among your other life tasks/subtasks that may threaten your ability to reach your goal. Describe how you will minimize the effects of these threats. Name specific actions you will take to reach your goal. Each action must be measurable over a specific time frame (e.g. meet 3 new people each week for the next month).
Step 6: Submit the materials from Steps 2-5 by the assignment deadline and Live your plan!!!!
