Assignment Overview
In Part 2, learners continue their development of the health promotion plan from Part 1, to demonstrate the application of each step of the health promotion process. The assessment incorporates target risk factors, population and Location characteristics, target health outcomes or indicators, and community resources. An overarching goal statement communicates the intended health improvement for the target population. Plan development integrates knowledge of health promotion theory, barriers and facilitators of desired health behavior, and the social determinants of health. In addition, learners identify guidelines and recommendations to support selected interventions, community resources, and potential partners to support plan implementation. Short-term
and long-term outcomes establish measures for monitoring plan implementation and evaluation of effectiveness.
Assignment Format
The required format for Part 2 is a PowerPoint presentation.
The presentation must contain a minimum of 13 slides but no more than 21 slides.
Each slide must include a heading and content that incorporates the required criteria for plan development.
You will not be required to record the presentation, only develop the content.
Creativity & Organization
Headings, bullets/numbers, and tables are helpful to organize information for the reader.
Font, colors, images, and graphics are valuable to engage the reader and reinforce the main messages.
Citations & References
Citations and references should observe APA Style.
Citations should be included within the individual slide(s) to illustrate where the source informed your work.
A minimum of six (6) references is required.
Keep in mind that you will need to include more than the required references from Part 1 to support your plan.
