Safe dose range- 20,000-40,000units per 24hours by continuous IV, 5.
Acceptable routes and rates (if applicable)- IV and SQ 6.
Any critical assessment data (physical, labs, other medications, procedurally, interactions) you need to evaluate prior to giving this medication- Bleeding risk PTT should be checked, platelet counts. 7.
How you will evaluate the effectiveness of the medication- by evaluating PTT labs and monitoring the client for s/s of clotting or increased bleeding. 8.
How you will educate the client BEFORE administering this medication- Education on bleeding/bruising risks, Checking with your Dr. prior to taking any other medications or OTC’s due to potential interactions (AVOID NSAIDS). 1.
Medication name (Generic)- Protamine Sulfate

What is the class of the medication? Anti-heparins
3.
Why is this medication appropriate for this client? Protamine Sulfate is the antidote for Heparin and would be used in the event of heparin overdose or toxicity. 4.
Safe dose range- Do not administer >100 mg in 2 hours without rechecking clotting values. 5.
Acceptable routes and rates (if applicable)- IV push (IVP)
6.
Any critical assessment data (physical, labs, other medications, procedurally, interactions) you need to evaluate prior to giving this medication- Clotting lab values (PTT), This med is contraindicated in pts w/ hypersensitivity to protamine or fish, pts who recently received protamine containing insulin and men who have had a vasectomy. 7.
How you will evaluate the effectiveness of the medication- Normalization of clotting factors in heparinized patients. Control of bleeding.

How you will educate the client BEFORE administering this medication- by explaining the purpose of the medication (controls bleeding/ reverses heparins effects on clotting factors), avoid activities that may result in bleeding (shaving, brushing teeth, rectal temps, ect) until risk of hemorrhage has passed.
