I am Nadine, the Clinical Social Worker with Pathways to Housing, and we will be working together to create and reach some of your therapeutic
goals. I want to introduce myself to make the client feel comfortable. Collaborating to create working alliance
TH: That sounds fine. SW: Before we begin working,
we will review some rules needed to make sure the work that we do together is I will cover them briefly so we
do not use too much of his session reviewing the rule and make her feel included in this Reviewing boundaries

TH: I never thought about it like that before. Maybe we can
add no judgment in therapy. I am glad she feels included, and I hope that motivates her participation.

SW: Do you feel comfortable discussing some of the circumstances that have brought you here for therapy? Probing Questions
TH: Well, I have been very depressed and I am not feeling it today, but some days I feel
like I want to kill myself SW: You have days when you think about committing suicide?

TH: Yeah, I have tried to take pills in the past, and I also tried to jump off a roof once But it didn’t work. They just hospitalized me and gave me more drugs to take. Wow, I am slightly shocked at the various methods this client has attempted to commit suicide with. SW: Wow that sounds very stressful on you. Do you think about suicide or have you developed a plan to follow through with your suicide? Validating feelings
Suicide Assessment TH: I just feel like everyone is better off without me around. I
have tried and cannot seem to get it right. I plan to hire a hitman once I save enough money. Sounds like the client is not very confident. I need to explore the “hit man” theory more. SW: Where do you plan to find a hit man for hire? Reality Testing
TH: I am not sure, I just know I am a waste of space. SW: well, I don’t feel that way. I think you are important to many people.

you believe people do not care about you? TH: for starters, I have been raped several times. Each time I felt like my safety, my life, my body… no one cared. SW: I am so sorry that you have gone through that. You said you were sexually assaulted multiple times? Restating the questions
TH: It was three times over the
last 7 years. I feel like every time I start dating a black man,
he abuses me physically or sexually. It is interesting that this client is a black woman who reports being afraid of black men. SW: I am so sorry that this happened to you. You did not deserve to be treated like that or to have to deal with something like this. Appropriate empathy
TH: Can I tell you something else? I am happy she is beginning to
engage more. SW: of course, you can tell me
anything! This is a safe space. I want to encourage this communication TH: I think I was cursed when
I was in middle school. That would explain all the bad things that have happened over
the years. SW: You say you were cursed in middle school, and this as Probing questions

TH: There were these girls who picked on me. I would get
bullied because of my weight. I think they put a curse on me because nothing’s been right since. This delusion of a curse has been with the client since middle school
SW: Wow, it sounds like you have experienced a lot of trauma during your life.
TH: You know what? I HAVE! Everyone thinks I just exaggerate and complain but I have had a lot of trauma. I am glad someone finally gets it. Corrective emotional response
SW: What I would like you to do is to think of some of the things that get you upset, or trigger you. Write them down and we will discuss them next week in session.

This interview was an initial session with a new client. In this interview, my focus was to build rapport, establish boundaries, and empower the client. The client was very sheepish while speaking about herself, but her tone and affect slightly changed when her feelings were validated by me as her therapist. She seemed more forthcoming, and comfortable. My initial objectives were achieved. The focus moving forward will be identifying her triggers and developing some coping mechanisms with the use of CBT and some mindfulness techniques (unofficial DBT). Additionally, I will support the client in developing a safety plan to acknowledge when she does feel suicidal. Agenda for Supervisory Conference
