Activity Plan Activity/Provocation Plan  Child’s Name: Veronica  Age Group: 24 months  Activity/Provocation Category: Small Fine Motor  Individual Developmental Goal for this Activity: Improve hand eye coordination and fine motor skills.  Activity/Provocation Name/Title: Building Blocks  Brief Description of Activity/Provocation: Have some blocks on the floor for Veronica and encourage her to stack them on top of each other to create a tower or house. She will develop fine motor skills as she continues to stack and holds the blocks. Her hand eye coordination will eventually improve as she gets the hang of moving the blocks around.  Child’s Name: Veronica  Age Group: 24 months  Activity/Provocation Category: Large (Gross) Motor  Individual Developmental Goal for this Activity: Balancing and Coordination  Activity/Provocation Name/Title: Outdoor Obstacle Course  Brief Description of Activity/Provocation: In the play area create an obstacle course. Include tunnels to crawl through, cones, hoops, and a zig zag line. She will need assistance to help move through, but this will help improve Veronica balance.  Child’s Name: Veronica  Age Group: 24 months  Activity/Provocation Category: Science/Sensory  Individual Developmental Goal for this Activity: Inquiring and sense of touch.  Activity/Provocation Name/Title: Sensory Bin  Brief Description of Activity/Provocation: Fill sensory bins with sand, rice, pom- pom balls, or pasta noodles. Encourage Veronica to use scoops, cups, or her hands to find what is hidden in the sensory bins.  Age Group: 8 months  Activity/Provocation Category: Language  Individual Developmental Goal for this Activity: Improve Communication skills.  Activity/Provocation Name/Title: Nursery Rhymes  Brief Description of Activity/Provocation: Pick a short nursery rhyme with hand gestures and movements. Encourage Antonio to interact and mimic the movement as it is being sung and performed. This will help develop communication, language, and social development skills.  Child’s Name: Antonio  Age Group: 8 months  Activity/Provocation Category: Science/Sensory  Individual Developmental Goal for this Activity: Sense of being curious.  Activity/Provocation Name/Title Sensory: Sensory Paint Bag  Brief Description of Activity/Provocation: Take paint and put it in a zip lock bag. Make sure the paint is nontoxic and zip lock bag is secure so he cannot open it. Put Antonio in the highchair and give him the bag with paint. Let him use his hands to squish the bag and rub it together. This activity will help with his senses and brain growth and support his curiosity.
