Family Action Plan
1.
Problems and Goals
a.)
Identify the family’s problems.
Mrs. Y is pregnant and having difficult problems. Mr. Y is worried about
financial burdens when the baby arrives. Mr. Y doesn’t want his wife give
birth in the hospital due to the been undocumented, the teacher is
concerned that their daughter is snot ready for kindergarten and lastly both
parents want their children to learn English.
b.)
Describe the family’s goals.
The family’s goal is to have their baby in the United States, get their daughter
ready for kindergarten, and for their children to learn English.
c.)
Make sure to include the cultural or societal factors that may influence the
family that you would need to consider. For example, what may be some of
the family’s cultural values and how do those values influence their goals?
What may be some current events or issues in your community that impact
how this family functions?
We have plenty of cultural values that will coincide with family goals. The Y
family value their privacy and does not want to share their any information
with anyone other than a cousin. Mr. and Mrs. Y value being able to help
themselves other than asking for help. They also value their children and
education and want their children to go to school in the United States.
2.
Team Members and Roles
a)
Identify and describe the members who will comprise your team.
(For example, my supervisor, lead or assistant educator, specialist or
social worker from ABC Agency and myself).
The lead teacher, the director, the family’s doctor, social worker, and I
Mrs. Washington from ABC Agency, are the members that will help the
Y family.
b)
Define each team member’s responsibility in implementing the plan.
(For example, the agency social worker is responsible for assisting
Mrs. Y to locate appropriate maternity care.)
To a plan in place the team will implement a plan that will provide
Mrs. Y with maternity care she needs, they will get pamphlets and
information they need on what they children will learn in the US
school system, they will be provides resources for their children to
learn English language, and they also will be provided with a family
support counselor to help them with any information or questions
they may have.
c)
Make sure to include any ethical issues you and the team may need
to consider. For example, how might the family’s undocumented
status affect how you plan to support them? Also, consider how your
beliefs and biases may influence your decisions. You may need to
conduct research about immigration laws and supportive
organizations in your state.
The ethical issues the team may to consider is the family
undocumented status and the privacy of the family information.
3. Tasks and Actions
a)
Create a list of the tasks and actions the team and family will
complete to resolve the problems.
Mrs. Y getting maternity care, the kids going to school, watching
English language tv and making friends in the neighborhood to better
their English language.
b)
Prioritize the task in the list.
Getting maternity care, going to school, English language tv, and
making friends.
c)
Include examples of materials and resources you would share with
the family.
The Y family will have family will have material resources shared with
them about maternity care, US school systems, as well as resources for
English language and counselor to support them.

How would you address communication challenges due to the
family’s language or cultural preferences?
I would address communication challenges due to the family’s
language or cultural preference is that the family does not share
personal information to people they don’t know.
5. Introduction of Plan
a)
What would you say to introduce the plan to the family?
Hi, I’m Mrs. Washington and I’m part of the team that will be working
with your family and on behalf of myself and others, I present you with
your family plan. I will explain to them that all the information they
need about maternity care, USs school system, English language, and
family support is in that plan. Also, I have included information on
myself as well as the team to ensure we are all here to help.

How would you encourage the family to follow the plan to meet
their goals?
I would encourage the plan by making sure Mrs. Y check in go to every
appointment, the kids go to school every day, they watch English tv
shows and playing outside with neighborhood friends.
6. Monitoring and Evaluation
a)
How will you monitor the family’s progress?
The family’s progress would be monitored on paper to keep their
private information protected.
b)
How will you evaluate the results to assess if the family reached their
goals?
I would evaluate the results by doing routine checkups on the family,
and making sure they have plenty of resources to reach their goals.
c)
In what ways would you follow up with the family after the plan is
completed to provide continued support?
After the plan is completed I would follow up with the family for the
first 6 months just to
make sure things are working on their behalf
and going smoothly and then I would follow up with them within a
year.
