Why did you select the assessment tool? Working with families and children as a foster care case manager I have the opportunity to see how family dysfunction destroys families. In most situations, the illegal use of prescription drugs and street drugs like Marijuana, Crack, or Cocaine is used as an attempt to cope with life. The victims of drug abuse usually are dealing with pain (mental or physical) and don’t have the resources to be resilient. It is my responsibility to assess the safety, health, and well-being of the family functioning. Using this assessment tool would help a case manager to make a decision about the safety of a child and services that may be needed to help the parent so reunification with the child can take place. This is why I choose to utilize the LEVEL 2—Substance Use—Adult* * Adapted from the NIDA-Modified ASSIST. This assessment identifies individuals receiving care have been bothered by “using medicines on their own without a doctor’s prescription, or in greater amounts or longer than prescribed, and/or using drugs like marijuana, cocaine or crack, and/or other drugs” at a slight or greater level of severity. Why might it be especially helpful for use with adults? Substance use disorder (SUD) is complex a condition in which there is uncontrolled use of a substance despite harmful consequences. People with SUD have an intense focus on using a certain substance(s) such as alcohol, tobacco, or illicit drugs, to the point where the person's ability to function in day-to-day life becomes impaired. People with a substance use disorder may have distorted thinking and behaviors. Changes in the brain's structure and function are what cause people to have intense cravings, changes in personality, abnormal movements, and other behaviors. (Saxon, n.d.) The DSM-5-TR Level 2—Substance Use—Adult is an adapted version of the NIDA-Modified ASSIST. The 15-item measure is used to assess the pure domain of prescription medicine, and illicit substance use in adults aged 18 and older. It is completed by the individual prior to a visit with the clinician. If the
individual receiving care is of impaired capacity and unable to complete the form (e.g., an individual with dementia), a knowledgeable informant may complete the measure. Each item asks the individual receiving care (or informant) to rate the severity of the individual’s use of various substances during the past 2 weeks. What challenges or limitations might there be for this assessment tool? A challenge to this assessment may be the client's memory, Brandell teaches, “It may be difficult to get reliable information about AOD (alcohol and other drugs) patterns or the taking of prescribed medications because the older adult’s memory is poor. If the older adult lives alone, there may be no one else to provide reliable information so assessment may take longer, requiring that the clinician be more patient”. Also, “Clients are often convinced that no AOD problem exists despite comments to the contrary from others in their lives. Involuntary clients are particularly determined to prove to the clinician that no problem exists. However, Brandell states, “The skilled caseworker avoids this trap by refocusing the assessment to provide an opportunity for the development of rapport and trust prior to discussing AOD issues” (Brandell, 3/2020, p. 419).
