Case Study Analysis: Christine Marissa Shumate HUS 412 9/27/23 I. Case Study Key Players Christine - 72-year-old white female Charlie – 76-year-old man, married to Christine. Meredith, 38-year-old, Christine and Charlies daughter, 2 nd grade teacher, lesbian. Dr. Carter- Christine’s psychiatrist for the last 5 years. II. Case Background Circumstances Christine is a 72-year-old female who is experiencing symptoms of depression. Christine’s behavior and health has changed over the last year. Christine experienced a heart attack 8 months ago and doesn’t follow up regularly with her physician. Christine is sleeping in later than normal and shows little or no interest in normal activities or social involvement. She attempted suicide by overdosing on medication 5 years ago. While Christine denies any suicidal ideation, she uses negative remarks towards her life such as “I would be better off dead.” Christine lives with her husband, Charlie, who’s health has also been decreasing lately. Charlie and Christine have one daughter, Meredith, who is in a serious relationship with a woman. This causes serious conflict between Meredith and Charlie, interfering with their relationship with Meredith. Another issue is Charlie and Christine’s overall health and ability to clean up after themselves and assure their needs are met. One concern is Christine not wanting to go to the doctor to avoid ‘any more bad news,’ Christine’s acceptance of help is a concern to ensure her needs are met. III. Environmental, Familial, Social Case Circumstances There are both positive and negative environmental, familial, and social factors surrounding this case. A positive factor is that Christine still has her family to support her, such as her husband and daughter. A negative factor is that Charlie and Meredith don’t get along well, so Meredith doesn’t always feel welcome to their home. Another negative factor is that Christine doesn’t really enjoy social settings anymore, which include church. Before Christine was involved with her church and lead Sunday school, now Christine shows little interest. The biggest environmental factor that is a concern is the cleanliness and upkeep of the home Christine lives in. This is a concern because of the decline in health in Christine and Charlie and their age, so they are likely unable to keep the home in good shape. IV. Legal Implications The only legal issue resolving this case would be if Christine was no longer able to speak up for her own medical care and needs. If Christine refused treatment that was medically necessary, she might need a medical power of attorney to ensure her well-being and safety. V. Community-Based Resources One resource that would be available to Christine in my community is in home care. I personally would recommend Home Instead Senior Care in Wilkesboro, NC. Home Instead offers in home personal care including meal prep and light housekeeping. I think this would benefit Christine in many ways, I think this companionship could help her
overcome some of her depressive thoughts. I also think that having someone to help do housecleaning would be beneficial. The senior center offers many activities, groups, and classes for senior citizens. I think it could really help Christine to get out of the house and interact with others and find something she enjoys. Case Plan and Recommendations My plans for this case would be to assess Christine for possible suicidal ideation and moderate depression with possible anxiety. If there are no signs of suicidal ideation, then I would talk with Christine about getting an in-home aid to help assist her with house cleaning and transportation. I think the in-home aid would be good to encourage Christine to get up more and get dressed and possibly get out of the house more. I would also like to talk with Meredith and Christine regarding a possible medical power of attorney in the future if needed. I think a strong support system is vital for Christine as well. I would like for Christine to become a member at the Wilkes Senior Center to try and find something she enjoys and make new friends. I would encourage her husband Charlie join with her as well as encouragement. Pros to my plan is getting Christine more involved throughout the community as well as building a stronger support system. Another pro is getting assistance needed at home and having less to worry about. Cons would be if Christine is willing to allow an in-home aid to help her and if she would be willing to try out the Senior Center. Also, possible family therapy with Meredith included. VII. Professional Skills The three skills that I think are vital to this case plan that I possess are supportive, sensitive to others, and understanding. I think being supportive is important because Christine needs the support regardless of whether she accepts your resources or not. Of course, I would strongly encourage them, but I would be supportive either way and reassure Christine that I am here to help her. I think being sensitive to others is also vital because I think Christine will respond better to a sensitive and caring approach than a harsh and demanding one. I think understanding is an important skill in general, however in this case I feel that Christine needs to feel understood instead of judged.
