Introduction: The FICA Spiritual Assessment model discusses the four domains in presence of faith/belief, importance of spirituality on an individual’s life and the influence that values have on a person when making decisions in the healthcare, the community that surrounds you, and they the way one wants their care to be addressed in the health care facility. I got the chance to take time and interview Linda, 54 years of age (my mom) and get a better understanding of her spiritual beliefs. This interview took place in our home at the dining room table. While proceeding with asking her these questions I tried to make it as professional as I could, therefore, I talked in a positive tone and had her elaborate on the yes/no questions to get more knowledge on the spiritual aspect for this assessment. Below are the questions along with her responses. FICA Tool F Faith, Belief, Meaning Religious/Religiosity Pertains to one’s beliefs, behaviors, values, rules for conduct, and rituals associated with a specific religious tradition or denomination (O’Brien, 1999). Spirituality Generally, an “individual’s attitude and beliefs related to transcendence (God) or to the nonmaterial forces of life and of nature...the dimension of a person that is concerned with ultimate ends an d values” and meaning (O’Brien, 1982, p. 88; Taylor, 2006). Do you consider yourself spiritual or religious? Patient is more spiritual, there are aspects of full religion that I do not believe in therefore, she is more spiritual. Do you have spiritual beliefs that help you cope with stress? Yes, I believe my gradian angels help guide me through the tough times in life my patient stated. What gives your life meaning? Patient stated her family, husband, and my kids give her life meaning. I Importance and Influence
What importance does your faith or belief have in your life? My patient believes that everything happens in your life for a reason because when one is born God has already planned out their life path. On a scale of 0 (not important) to 5 (very important), how would you rate the importance of faith/belief in your life? Patient would rate my faith/belief around a 4 and in certain situations possibly a 5. Have your beliefs influenced you in how you handle stress? Yes and no, hard to focus on my beliefs when my mind is stressed, but I do look for guidance or signs through it my patient mentioned. What role do your beliefs play in your health care decision making? Patient stated her beliefs are major due to working in the hospital setting so I see it all go wrong or right. C Community Are you a part of a spiritual or religious community? No, never settled in one that I found fit myself my patient said. Is this of support to you and how? Yes, my community gives me stability along with encouragement during hard times and good times my patient said. Is there a group of people you really love or who are important to you? Yes, friends are huge my patient said. A Address in Care We have talked a lot about your spirituality and/or religious beliefs and how they may or may not be of help to you during your illness. How can your health care providers best support your spirituality? Patient stated that her health care provider can best support by listening and caring out the patients believes and wishes without bringing family members interferences. How would you like your health care provider to use this information about your spirituality as they care for you? Patient stated she would like my health care provider to go in depth about the procedures and my beliefs to work together for an outcome that benefits me along with my spirituality/beliefs.

Summary: After discussing with my patient, it is known she is more spiritual than religious. Family, friends, her husband, and her kids are her biggest support system and the reasoning behind her life having meaning. When it is time to act with her care in the health care facility she wants to be listened to and to have the procedures deeply explained. Following her demands without having her family or kids intervene with those spiritual decisions during care is a major aspect to fulfilling her spiritual/religious belfies makes patience trustful and comfortable in their care.
