CASE STUDY 6 3 Case Study Six What indicators say it may be helpful to use a mental status exam before proceeding? The client’s disheveled appearance and vague responses to questions are indicators that a general mental status exam would be beneficial. A mental status exam will provide an assessment of the client’s baseline functioning as well as the effects the crisis event has on the client's functioning (Miller, 2012). The client's appearance and responses could indicate a high- stress response to a trauma event or it could be an indicator of functioning level. The client could have a traumatic brain injury that has limited their functioning and general orientation or could be diagnosed with IDD and the appearances and responses are the client’s baseline functioning. By assessing the client's general mental status it allows me to assess the need for trauma services, emergency medical services, or the need to provide services more appropriate to the client’s baseline functioning level. In this case, I would likely utilize the Short Portable Mental Status Questionnaire (SPMSQ) (Miller, 2012) as it is simple and quick so I can quickly get an understanding of the client's baseline functioning and assess the need for additional specialized assessments. I wouldn’t think utilizing the Trauma Symptom Inventory (TSI) would be appropriate due to the vague responses the client is already providing I think this assessment may be a little overwhelming and may not provide enough general information about the client to build a clear picture of the client’s functioning and how it has been impacted by the crisis event (Miller, 2012) . How would you integrate the exam into your session? The SPMSQ has written prompts to ensure the scoring of the responses is consistent (Pfeiffer, 1975). I would ensure to explain that I am going to ask some questions to see how well

CASE STUDY 6 4 the client can remember things. I would ensure to explain the need for the assessment and ask the client if they are willing to participate as assessing a client should be done with their consent. I would then clearly ask the questions and encourage the assessment even if the client is unwilling to answer some of the questions. I would ensure to quickly move on to the next question, so the client does not feel overwhelmed or interrogated. If appropriate, which aspects of the exam would you especially focus in on with your client? I don’t think it would be appropriate to focus on one area of the assessment. I think that would unintentionally skew the score towards whatever my assumed cause of the client’s disheveled appearance and vagueness could be. I think that by following the tool completely without prompts or input will allow me to see the complete picture and assess the client and their needs clearly.
