2. Deontology is the concept of duty and respect for persons to define appropriate ethical action. This is the action is considered morally good because of some characteristic of the action itself, not because the product of the action is good. There was a situation that did not correlate with deontology. For example, a patient needed a heart transplant, and the organization was not going to cover it because heart transplants were not covered under the insurance. The patient needed the heart transplant and the provider stated that this is a true death sentence. After Dr. Peeno stamped the claim as denied, the other employees congratulated her even though this was not considered morally good. Instead of looking at the big picture, the life of a patient, Humana was wrapped around the idea of saving money, not saving a life. Instead of utilizing half a million dollars on a heart transplant, they used it on a sculpture. When Dr. Peeno noticed that there were cases that were being denied after she approved of them, she stated to her assistant that if she did this again, she would be fired. She started to go to an ethic class which started to promote her voice. For this, the case where the woman who lost her voice due to a stroke, a doctor put a request for a voice machine. Dr. Peeno had him come in and ask why this would save their company money. With knowing what the patient is feeling, this would help reduce the visits, medications, etc. After hosting this meeting and everyone listening to this, Dr. Peeno approved the machine, which was using deontology. 3. Biomedical ethics includes the concepts of autonomy, nonmaleficence, beneficence, and justice. Beneficence is the principle of doing good or acting in the best interest of the patient. This is often applied to healthcare decision-making, and it requires healthcare professionals to consider the potential risks and benefits of any course of treatment before any course of treatment before proceeding. After approving the case for the woman who needed a voice machine, Dr. Peeno started to use her voice more. She was asked to be on the news and discuss the issues with a case regarding a baby boy. Their insurance company stated that
they were not able to transport the baby by an ambulance and sent them to a hospital that was 50 minutes away, but he did not have that time on his hands. They went to the nearest hospital, and to save his life, they had to amputee all 4 limbs. Dr. Peeno stated on the news what HMOs are starting to do with trying to save the company money, rather than help society regain trust in the healthcare system and decide based on ethical decisions, rather than trying to save money. She used her voice to state that this situation was not beneficence, and that HMOs are not acting in the best interest of the patient, but rather the best interest of the business. 4. Authority based ethics is the decision about ethics that are based on the central authority such as in a theology or an ideology. With authority based ethics, Dr. Peeno refused the heart transplant because Humana, the HMO organization, that she was employed at wanted her to. Instead of approving it like she wanted, she denied it to comply with the organization's wishes. With this being said, this is not patient- centered care at all since the patient passed away due to the "burden" that this procedure would have done on the company.
