1 BRN Discussion Questions 1. Based on this case list all the violations noted by the BRN against this nurse. The nurse failed to document the total amount of Dilaudid as required by the Mission Hospital's Medication guidelines. Failed to administer the second dose of Cefazolin to patient #2. The nurse falsely documented that the indwelling urinary catheter was discounted according to the order of the physician who had ordered its removal. Failed to conduct blood transfusion for patient #4 because the patients' blood counts were normal, and she saw no urgency in doing as was ordered. Falsely documented that patient #5 was on telemetry monitoring. Falsely documented administering Percocet for the patient. The nurse failed to restart an IV to administer fluids and an antibiotic for patient #7. Did not document her failed attempt of restarting the IV. The nurse failed to perform a morning POC finger stick to assess glucose level as ordered. The nurse did not follow the orders of the physician that required her to apply an SCD and TEDs for patient #7. The nurse did not document intake and output for patient #7. o Do you agree or disagree with these findings? I agree with the findings. o Were you surprised by this case? Yes, I was surprised how the nurse failed to adhere to his duty as required. Besides, it is surprising how she lied on several occasions and failed to give the right medication to the
2 patients. The nurse failed to ask for help on how to restart the IV. It is heartbreaking how the nurse failed to follow the instructions and conduct her work appropriately. Putting on SED ad SCD is not a difficult thing to do. 2. Do you feel that the institution this nurse worked for was justified in it's disciplinary counseling's and follow-up actions with the BRN? o Yes/No (Why?) I feel that the institution the nurse worked for provided enough time and counselling for her to do what is required as a nurse. I think the institution provided disciplinary counselling and gave the nurse the required advice on how to improve her work performance. Besides, the hospital must have conducted follow-up before deciding to act. Patient health should be a priority for all healthcare institutions, and thus am convinced that the hospital did what it could to ensure the nurse performs her duties well. o Was there anything in the case that you disagreed with? The BRN decision is justified. The nurses are required to follow the policies and procedures of the institution they work for, particularly when they pertain to the patient's best interest. The hospital policy should be adhered to to ensure the best services are offered to the patients. Healthcare providers need to avoid assuming when it concerns the wellbeing of the patients. For instance, although the nurse did not see the urgency in conducting a blood transfusion because the patient's blood counts were normal, it would have been better if she had done as ordered by the physician. o Do you feel the institution was too quick to notify the BRN, or too slack? I feel the institution was not too quick to notify the BRN. The nurse was given enough time to change her behavior. I would wish the institution to give me a chance to improve on my
3 performance if I make a mistake. This can help me to improve and ensure quality services are delivered to the patients. It would have been demotivating if the hospital had notified the BRN due to a single mistake. However, the nurse made several mistakes prompting the hospital to notify the BRN. o What might have happened if the facility had not filed a complaint with the BRN? Failure of the institution to file the complaint with BRN would have made more patients to become the nurse's victims. Due to the high number of mistakes made, the nurse was not ready to change so that she could perform her duty as required of her. 3. List all the sanctions/conditions imposed on this nurse by the BRN . o The BRN required the nurse to obey all laws. o Comply with the probation program of the board. o To provide written notice to the board within 15 days regarding change of residency, practice, or licensure outside the state. o Submit written reports required by BRN. o The nurse to engage in registered nursing practice in California for a minimum of 24 hours every week for six consecutive months or as required by the board. o The nurse should obtain prior employment approval from the board and report all the employment requirements. o The nurse to obtain prior approval from the board concerning the level of supervision before starting any employment as a registered nurse. o The nurse's employment shall be limited. o The nurse to complete a nursing course at her own expense.

4 o The nurse to pay the board the costs of $9,621.75 associated with investigation and enforcement. o The nurse may surrender her license to the board during her probation term if she ceases to practice due to health reasons or retirement. o The BRN can suspend the nurse's license if she violates probation. o Will this nurse be able to work? The nurse will be able to work. o Do you feel these sanctions are in excess or justified based on the violations identified? I feel the sanctions were justified to ensure the nurse's performance improved. Close monitoring will ensure the nurse adheres to the code of conduct for nurses. However, I think maximum supervision should be minimized as the nurse's performance improves with time. 4. If you were one of the patients identified in this case, how would you feel? I would feel bad and disheartened with the services offered by the institution. I believe that some patients would have developed additional complications and even stayed longer at the institution due to worsened conditions caused by the incompetency of the nurse. o What would be your general impression of the nursing profession? My general impression about the nursing profession is that nurses should consider the wellbeing of the patients to ensure they are safe. If I have no medical background, I can trust the nurse in taking care of my health condition. o What would you think of this nurse? I would think of this nurse as incompetent healthcare professional. I would not recommend that she take care of the patients unless she completes her nursing course.
5 o Would you litigate the institution? The nurse? Why? I would litigate the institution and the nurse if I were severely injured. I do not need to be punished due to the nurse's incompetency. The hospital should be responsible. 5. Does this case affect your thoughts regarding your practice as a nurse? This case affects my thoughts regarding the nursing profession. It gives me the opportunity to learn from the mistakes made by other nurses in the field. I want to be a competent nurse who prioritizes the patients' health. o Identify no less than three (3) elements of practice you will be mindful of and why. Ensuring the patient has received all the medication. This will ensure the patient's speedy recovery and avoid incidences of the patients missing to take their medicines. Documenting all the practice conducted as a nurse. Documenting will help to follow up on the patients easily and the practices conducted. Asking for help. I would request help if I am unsure about carrying a particular function since nursing entails teamwork. o If you had received report from this RN, what steps if any, would you have taken upon discovering her errors? Consider available internal and external options. I would have informed the nurse in charge and asked what I should follow. I could also call the nurse and talk to her in person to advise her on what she should do to ensure the safety of other patients. o What legal and/or ethical principles underlie the nurse's obligation to the patient? Which, if any, would govern your decision-making process?
6 Beneficence, non-maleficence, and fidelity are some of the ethical principles that underlie the obligation of the nurse to the patient. Nurses are supposed to do good and what is right for the patients. 6. What are three (3) main take-aways you have learned from this case and that you will incorporate into your practice? Working as a team with my co-workers in the future as a nurse is essential since nursing is dynamic. I have also learned that following the physician's orders is good. Telling the truth is essential as a nurse. Since everyone is prone to mistakes, I will ensure, to tell the truth in case I make any mistake.
