Furthermore, the patient has already stopped taking her medications due to affordability issues. The FNP should prioritize the patient’s safety and well-being while addressing the underlying issue of medication affordability. The FNP should assess the patient’s immediate risk by asking more detailed questions about her current emotional state, such as whether she has thoughts of self-harm or suicide. If the patient expresses any intent or plan to harm herself, the FNP should take immediate action to ensure her safety, which may include involving emergency services or a mental health crisis team. However, it is important to engage the patient in an empathetic and supportive conversation by actively listening to their concerns and feelings. It is important to validate her feelings and assure her that help is available. As part of the intervention, the FNP should educate her on the significance of continuing her medication and the associated risks of discontinuation without proper medical guidance. The FNP can explore various options to address the affordability issue, such as connecting the patient with medication assistance programs or recommending affordable, generic alternatives (Pickett et al., 2023). The FNP can provide information about patient assistance programs offered by pharmaceutical companies or nonprofit organizations. These programs often provide free or discounted medications to individuals who meet specific criteria. It would also be beneficial to explore state and federal prescription assistance programs that may be available to help low-income individuals afford necessary medications. Ultimately, the FNP should work collaboratively with the patient to develop a plan that addresses her immediate emotional distress, ensures her safety, and addresses the medication affordability issue. Referral to a mental health specialist or counselor may also be appropriate to provide ongoing support for her depression.
