Case Scenario: Shawn Billings, a 28-year-old African American patient, comes into the clinic today. He has been deemed a “frequent flyer” by the staff at the clinic and was at the clinic last week and 4 days ago with a migraine, given a shot of Toradol and Ativan, and sent home. He is here again today for an extreme headache. He is very agitated today. He is here with his father and is worried that he will not get any medication. Healthcare: Exploring the Impact of Socioeconomic and Social Determinants Black individuals tend to encounter socioeconomic challenges that hinder their inclination to seek medical assistance. Socioeconomic barriers encompass insufficient insurance, difficulties in scheduling doctor appointments while working, and a lack of confidence in healthcare providers. Eley et al. (2021) reveal that African American men are less likely than other populations to actively pursue medical care or preventive healthcare options. A considerable number of black individuals turn to their faith for healing and restoration. African Americans have traditionally been encouraged to draw on their religious faith and spiritual rituals as a means of dealing with the challenges and difficulties they face in life (Siler et al., 2021). It is regrettable that individuals belonging to minority groups, particularly black individuals, face a higher likelihood of experiencing serious health issues due to their lifestyles. Where they live can also influence their way of life. For instance, residing in a low socio-economic neighborhood with prevalent drug usage can contribute to substance abuse problems. Additionally, limited access to stores selling nutritious food nearby can lead to disparities in health, such as an increased risk of developing heart disease. The environments where we live, work, and engage in leisure activities have a significant impact as social determinants of health (SDoH). The SDoH covers a range of factors that affect our well- being. These factors include housing, access to resources, educational opportunities, neighborhoods, and communities. They have a direct or indirect influence on our overall health and wellness (Clay et al., 2023). The healthcare provider needs to establish a comfortable relationship when evaluating these aspects of social determinants of health. Communication with the Patient To ensure a trusting and nonjudgmental interaction, it is crucial for the healthcare professional to approach the black male patient with sensitivity. Everyone is characterized by variables such as social class, race, age, and gender, which can hinder effective communication if there is no mutual effort to attain knowledge and understanding (Ball et al., 2023). Building rapport by finding common ground can create a more comfortable environment for communication. Rather than directly probing smoking or alcohol use, it may be better to educate the patient about the harmful effects these substances can have on the body, which the patient may find less intrusive and more appreciative. Research indicates that offering advice, no matter how small, in a respectful and genuine way can be advantageous with smoking cessation (Ball et al., 2023). When gathering information about this patient's health history, I would ask these five targeted questions: Can you tell me more about your headache symptoms? When do they start? How long do they last? Do you have any other medical conditions besides migraines? Does anyone else in your family have a history of migraines? Do you smoke, drink alcohol, or use recreational drugs? If so, what and how much? How would you describe your lifestyle?
