2 Drug Abuse/Substance Abuse Drug abuse or substance abuse refers to the use of chemicals that change the functioning of the nervous system therefore affecting one's cognitive reasoning, perception of things and mood. The majority of these drugs are hazardous to the human body and can have health complications when used for a long period of time. Marijuana, cocaine, methamphetamine and heroin are commonly used drugs in the US and generally all over the world. Critical Health Condition Associated with Drug Abuse or Substance Abuse Drug abuse or substance abuse of illicit products such as Cocaine, methamphetamine, and heroin among others have problematic patterns of use ranging from mild to severe. Use of these drugs for a long time will lead to a condition known as substance use disorder (SUD). Substance use disorder causes distress to the drug user. People with the condition of substance use disorder have an overpowering desire to use these addictive substances and they look withdrawn from all other aspects of life such as association when they have not used these drugs. Many of these drug abusers have more than one substance use disorder at a time. For instance, one would want to take Marijuana and cocaine or heroin and Methamphetamine at the same time. Many of these drug users with substance use conditions have their health deteriorated and their families broken. Incidences and Prevalence of Substance Use Disorder SUD in US. The United States of America like any other continent in the world has reported a prevalence of Substance use disorder (SUD). A recent study showed that ”In a population of 46.3 million people, 16.5 % of the population had a substance use disorder in past years” (Brust- Sisti, et al., 2023, p.1). The 16% of substance use disorder is a very high percentage that the US
3 government has to seriously deal with, the good news is that substance use disorder is a treatable condition. Youth are more affected by the substance use disorder than the adults (Grant, 2004). How Substance Use Disorder Condition is Diagnosed, Monitored and Treated. A person with substance use disorder undergoes detoxification or withdrawal therapy. One can undergo detoxification as an outpatient or inpatient depending on the severity of the condition. Substance use disorder patients requiring emergency treatment are injected with a drug called Naloxone or Narcan Kloxxado. Medicines do not completely help in treating opioid addiction but they help in the recovery process (Teeters et al., 2017). After treatment, one is encouraged to undergo behavioural therapy by psychologists who offer counselling services. An ongoing treatment such as visiting medics for guidance and assessment should continue and finally attending counseling and self-help programs should continue for substance use disorder. Describe the Interdisciplinary Team that will Participate in Coordinating Care. Specifically, Define the Role of the APRN in Management and Coordination Advanced practice registered nurses (APRNs) form part of the interdisciplinary team used in treatment of the substance use disorder patients. The Advanced practice registered nurses unlike the doctors are friendly to substance use disorder patients and in most cases, these patients open up to these nurses during their recovery process unlike they do when attended by the doctors (Teeters et al., 2017). The services of these nurses are needed in administering drugs to patients experiencing severe withdrawal effects. Can Care Teams use the CCCR Model? 4 In the treatment of substance use disorder patients, the healthcare team should use care coordination clinical reasoning (CCCR) because of the chronic nature of the illness. This model will help the healthcare workers especially the advanced practice registered nurses (APRN) to navigate the complex patient care needs and offer the best medical and management solution to the substance use disorder patient. The Cost of Managing Substance Use Disorder in My Home County Treatment of substance use disorder (SUD) is quite expensive ranging from the time given to the patient to recover to the resources that are used up to the full recovery time of the patient. In the county of Los Angeles, close to a million patients had substance use disorder as of 2017. However, the treatment for these patients ranged from patient to patient based on the severity of the disease and the patient's recovery time (Brust-Sisti et al., 2023). What Resources are Available to Help Manage the Costs of Care Related to this Condition? There is a challenge in the treatment of substance use disorder as each state or country has its way of treatment. However many states, cities and countries have established inpatient rehabilitation programs. Others have offered community-based social model public sector programs. Besides, hospitals have assembled the right medicinal professionals to help in the treatment of substance use disorder patients. Identify Barriers or Challenges that you foresee for Patients and Care Teams. How can Barriers be Overcome? Substance use disorder patients after recovery face the social problem of being isolated and therefore end up lacking confidence in life. Most of these substance use disorder patients get
5 back to substance use. Another challenge these patients face is on the treatment, most of the patients cannot afford to pay for the cost of treatment (Teeters et al., 2017). Patients suffering from substance use disorder sometimes can’t access resources or evidence-based information. Healthcare providers too have challenges of limited resources and personnel and insufficient capacities in hospitals for the treatment of substance use disorder. All these barriers can be overcome by equipping health centres with medicine and specialized health personnel. Treatment of substance use disorder should be made free or affordable to the patients and finally, there should be community programs and facilities designed to help these patients suffering from substance use disorder (SUD).
