Reflection Questions for the BBC video Addiction: America’s Opioid Crisis
Before viewing the video, please read the article from Harvard Medical School, found in Content in the Mental Health Chapter section, entitled The Addicted Brain. Think about how it describes the process of addiction, withdrawal and relapse. Think about how the brain stores details of nearly every life experience including visual, olfactory (smell), touch, etc. and what a person did to achieve the experience. Understand that addicts want to avoid the effects of withdrawal at almost any cost. Also, understand that opioids include certain prescription pain medications and also heroin. Fentanyl is a synthetic opioid that is legally utilized in controlled environments, as an analgesic to control cancer related pain in patients that have built up resistance to other pain medications. It is many times more effective than Oxycontin, Oxycodone, Percocet, and far more potent than heroin. Please respond to these questions after carefully viewing the video, and post your answers in the Assignment Dropbox for this Assignment. This assignment is not just about memorizing content. You must use your ability to observe and use critical thinking. Suggestion: Take notes.
1.
Watch the film carefully and try to Identify 3 or more factors in the external environment that may be responsible for opioid addiction in the communities in the film. (Note: these factors could be behavior of health care providers, regulatory authorities, socio/economic factors, law enforcement, etc.). Are there any health care policy initiatives that could facilitate changes in these factors? One factor I see is that the opioid epidemic is caused by the medical care system due to overprescribing medications. Another factor in external environment would be the lack of access to non-opioid pain management such as physical therapy or counseling. Opioid addiction may be increased in specific areas by high rates of unemployment, poverty, and restricted access to mental health care. alterations to medical procedures and initiatives to deal with the societal and financial factors that influence addiction.
2.
Identify attitudes, behaviors and other traits of the individuals with opioid addiction depicted in the film. Google “Learned Helplessness.” Could it have been one of them? If so, why? attitudes of people with the addiction is that they just want to keep using. Some of them want to stop and
get help but they find it hard without any continued support. Learned helplessness could have also been of them because most of them tried to be clean and get help but the failed so they don’t try anymore to receive help and just live with the addiction 3.
Purdue Pharma (with its product Oxycontin) is the most often discussed pharmaceutical company in the context of the opioid crisis in the U.S. Observe and identify the unusual/extra aggressive marketing tool employed by the company and the Sackler family? It wasn’t just their salesmen. The widespread misuse and addiction to OxyContin that resulted in overdose deaths and opioid addiction was partly caused by Purdue Pharma's marketing strategies. They made extremely false safety claims, including the unfounded assertion that the extended-release version required fewer doses and was less likely to cause addiction. They also influenced medical opinions in prescribing the medications With benefits for doctors who prescribed
OxyContin more frequently, they encouraged physicians to write prescriptions for the drug. This method added to the drug's over prescription. 4.
Is addiction to prescription pain medications a by-product of medical errors in the US health care delivery system, unusual extra-aggressive marketing strategies, poor attitudes and perceptions about pain and injuries on the part of athletic personnel, or other factors? Addiction to prescription pain medications is the result of all these different factors seeing as they all play a part in fueling the problem in the united states
5.
Kevin Morse was the person (the voice over) who, near the beginning of the video, narrated the scenes about his previous life as an addict. How was his newborn child affected at birth and shortly thereafter? Pay attention to his description of his own behavior when, with his child in the car, he tried to make a buy from a dealer and discovered he’d been cheated. Comment on the reason for his reckless behavior. What drove him? His newborn child was affected at birth because he started shaking as soon as he was born which they called withdraw symptoms, so he
started feeling guilty thinking of hoe he affected his son as a baby. When he tried to buy from a dealer and got cheated I believe the reason for his reckless behavior Is the fact that his body was
so accustomed to using and all he felt in the moment is to use so that fueled his anger. 6.
Much later in the video Kevin Morse had become an “interventionist” when he was introduced to the Mandeville family and their 30 year old son, Derek, who had been an addict since 2005. Since then, his parents had been trying to help him but without success, in part, because Derek insisted on self-help, which didn’t work, eventually leading to an overdose event.
Why do you think Derek’s parents were unsuccessful and why was Kevin Morse successful in reaching Derek? I think Kevin Morse had more success in reaching Derek because he’s been through the same thing so he’s able to relate with Derrek more on that level in order to help him get out of that state.
In the past, did Derek and his parents “see” each other objectively when his parents tried to reach him? Only if you can, try to explain this dynamic. I'm going to say no they
didn't see each other objectively because they kept trying to get him help but couldn't understand what he was going through and feeling so he kept overdosing. But his parents have been there every step of the way for him addiction began
Do you think that Kevin’s previous experiences with addiction was a factor in his success
as an interventionist? If so, why? Yes because since he has experienced this before he has better ways to help people deal with this
What do you think about the inpatient intervention that Kevin promoted to Derek Mandeville’s parents which they helped to facilitate? I think this will help a lot especially with people struggling with this addiction because it can help them get over it especially since also Kevin has been through something like that he's able to get them more help because he can understand the position they are in
Do you think that an intervention that includes inpatient rehabilitation treatment works for all types of people with opioid addiction? Yes I believe that should help a lot of people with opioid addiction, especially those with severe addiction or complex demands, it can be quite helpful. However, choices about a patient's course of care should be made on an individual basis, taking into consideration their preferences and unique circumstances. A thorough evaluation by medical specialists can assist in identifying the best course of action for a specific patient.
7.
The film very briefly hints at protocols limiting the amount of pain medications that can be prescribed to an individual. Why is the increase in the popularity of Heroin a result of the change
in policy? The increase in the popularity of heroin as a result and a change of this policy is because of the fact that since people can't get the prescribed medication for the pain as they used through their results into something that gives them the exact feeling and even more so that's why it increases the use of heroine
8.
Describe the conflict of interest that still pervades the relationship between the Food and Drug Administration (FDA) and the Pharmaceutical industry that’s highlighted in the video?
The impact of the pharmaceutical industry on the FDA's policy-making procedures and decision-
making processes has been the focus of continuous discussion and research, despite the fact that the FDA is vital to guaranteeing the efficacy and safety of pharmaceutical goods. Stricter conflict of interest laws, greater transparency programs, and closer examination of FDA decision-making and procedure to guarantee that public health and safety come first are all attempts to resolve these conflicts of interest.
