With gloved hands, examine the patient by
using a bladder scanner or by palpating the
patient’s bladder for distention.
3.
Raise the bed to a comfortable working height.
Adjust side rails accordingly.

Lower the head of the bed to a comfortable
position

Positioned the patient in the dorsal recumbent
position. Asked the patient to relax her thighs.

Placed a waterproof pad under the patient and
draped the patient for privacy.
7.
Applied clean gloves, cleanse labia and urinary
meatus, working from the clitoris toward the anus.

Positioned a trash can nearby or placed a bag
between the nurse and the patient. Perform hand
hygiene and applied clean gloves.
9.
Obtained an indwelling catheter kit. Opened
the outer wrapping of the kit by tearing the
designated edge of the plastic wrap. Placed the
empty outer plastic wrap near the end of the bed
and used it for waste disposal. Placed the inner
catheter tray kit on a clean surface.
10.
Open the inner sterile wrap using sterile
technique. Using sterile technique, opened the top
flap outward to the far side of the kit. Then

opened the side flaps without bringing the arms
back over the sterile packaging. Opened the last
flap toward yourself. Noted that the tray was
sitting open on its own sterile field.

Draped the patient’s perineum appropriately:
Picked up the square drape and allowed it to
unfold without touching unsterile surfaces.
Allowed the top edge of the drape to form a cuff
over both hands. Placed the drape with the shiny
side down on the bed between the patient’s
thighs. Slipped the cuffed edge just under the
patient’s buttocks while asking her to lift her hips.

Applied sterile gloves. Picked up fenestrated
sterile drape. Draped this item over the patient’s
perineum, exposing her labia. Do not allow drape
to touch the unsterile surfaces.
13.
Placed the sterile tray on the sterile drape.
Arranged all the supplies on the sterile field,
maintaining sterility of gloves.
Opened the packet of lubricant
and
squeezed it out onto the sterile field.
Opened the package of sterile antiseptic
solution. Poured the solution over the sterile
cotton balls.
Opened the inner sterile wrapper of the
catheter.
If using a closed system kit, remove the
tray with the catheter and the attached drainage
bag, and place them on the sterile drape. Made
sure the clamp on the drainage port of the bag was
closed. If using and open system, attach the
catheter to the drainage tubing.
Lubricated the catheter.
14.
Cleansed the patient’s urethral meatus.
Separate the labia with the fingers of the non-
dominant hand
to fully expose the urinary meatus.
Use forceps to hold one cotton ball or swab stick at
a time.
Cleanse the labia and the urinary meatus from the
clitoris toward the anus.
Use a new cotton ball or swab for each area.
Appropriately dispose the used cotton balls or
swabs without contaminating the sterile field.

15.
Picked up and held the catheter 7.5 cm to 10
cm from the catheter tip, with the distal end of the
catheter loosely coiled in palm.
16.
Inserted the catheter:
Ask the patient to gently push the perineum down
gently, while slowly inserting the catheter into the
urethral meatus.
Advance the catheter until urine flowed through
the tubing.
After visual confirmation of urine into the tubing,
continue to advance the catheter.
17.
Release the patient’s labia, while maintaining a
secure hold of the catheter with the non-dominant
hand.
18.
Inflated the catheter balloon with the amount
of fluid designed by the manufacturer:
With the dominant hand, connect, connected the
prefilled syringe to the injection port at the end of
the catheter.
Slowly injected the total amount of solution.
After inflating the catheter balloon, released the
catheter. Gently pulled the catheter until resistance
was felt. Then advanced the catheter slightly.
Removed sterile gloves and applied clean gloves.
Connected the drainage tubing to the catheter.
19.
Secured the indwelling catheter with a
catheter strap or other securement device.
Left enough slack to allow leg movement. Attached
the securement device just at or above the
catheter bifurcation.
20.
Secured the catheter tubing to the upper
medial aspect of the patient’s inner thigh, allowing
enough slack in the catheter tubing to prevent
tension.
21.
Positioned the drainage bag lower than the
bladder by attaching it to the bed frame.
22.
Coiled any excess tubing on the bed and
fastened it to the bottom sheet with a clip or other
securement device. Checked to make sure the
urine flow was unobstructed, and the tubing was
not kinked.
23.
Provide hygiene as needed. Position for
comfort and ensure that all personal items are
