You live in Shiprock, New Mexico – in what is called the Four Corners Region -- and hold a professional license to practice as a physician.  You would like to open satellite offices and offer services in the neighboring states of Utah, Arizona, and Colorado.  Can you?  What federal and state agencies will you need to contact?  Does it matter if you are an allopathic or osteopathic physician? As a licensed physician in New Mexico I may only open satellite offices in the neighboring states if I, the physician, obtain licensure from each of the other 3 states individually; The only exception to this rule are limited to border states, for consultation or emergency (Kocher, 2014). The disconnected system currently in place makes it difficult for telemedicine use across state lines as well as in person type services. The U.S. federal government only provides guidelines for medical training and testing, each state they practice in has its own licensing board (Kocher, 2014). The push to use telemedicine and other technology to reach patients that otherwise would go without, led to state boards embracing the idea of an interstate licensure process that can help streamline the multi-state licensure process. In 2013, state medical boards discussed the introduction of the Interstate Medical Licensure Compact (IMLC) which significantly streamlined the licensing process for physicians attempting to practice across state lines (Interstate Medical Licensure Compact, 2020). Furthermore, in the scenario presented my original licensure was from New Mexico whose state board is not participating the IMLC. However, according to the IMLC webpage the three neighboring states of Utah, Arizona, and Colorado are member states (IMLC,2020). Therefore, in order to open my satellite offices, the easiest path I could take would be to obtain licensure in one of the three neighboring states. Once I have established this licensure and have successfully moved 25% of my practice to that “Compact member State” I would qualify to apply under the IMLC and can then add the additional 2 states I needed within a year of my application date (IMLC,2020). This would allow me to easily navigate opening satellite offices in all four states systematically and use telemedicine services if desired throughout the entire four corners region.
