Coding Skills Assessment Main Terms and Sequencing And 3M Encoder Coding Clinic Worksheet Tips Tips for completing the 3M Encoder Coding Clinic Worksheet: Read assignment instructions step by step. The assignment is much easier than it looks. Follow the steps to access the 3M Encoder through the Virtual Lab via the account you set up in Coding Foundations with your account user and password you set up. Follow the steps in the assignment step by step in the 3M encoder to access the AHIA Coding Clinic Resource within the encoder and take screen shots with each step to put in the boxes as your answer. That is Part 1. Part II you will follow the steps for accessing the information indicated within the Coding Clinic for each quarter indicated. You can copy and paste the requested information, or you can take a screen shot. Do not forget the second part of each question in Part II which is to look up the information in the Code Book, give the page and any instructional notes for each of the Coding Clinic topics you choose. Find the focus of treatment to sequence first. The sequence by order of listing in the documentation. 8. Main term is “sepsis” 9. Main term is “HIV” and see the Live Classroom and Chapter Specific Guidelines. 10. Main term is “hepatitis”. NOS means “not otherwise specified” or unspecified. 11. Main term is “sepsis”. Make sure you include the “due to” in the code. See live classroom for coding the shock and sequencing. 12.Main term is leukemia. 13. Main term is either “carcinoma” or “neoplasm, malignant”. See Live Classroom for what “in situ” means. 14. Main term is either “melanoma” in the alphabetic or “neoplasm, malignant” in the Table of Neoplasms with site being the skin and then where the skin is located on the body. 15. Main term is “neoplasm, malignant” and then the site as pylorus. Determine the type if using the Table of Neoplasm. See Live Classroom. You are also coding the anemia and main term is “anemia”. See the instructional note for sequencing. 16. Main term is “goiter”.
17. You have diabetes mellitus indicated as type 2. You code the DM2 with main term “diabetes”, complication of hyperglycemia and DM2 with mild degree malnutrition. “Long term” is the main term for the insulin use. 18. Main term is “syndrome”. And then “diabetes” as this is DM due to an underlying condition. Instructional notes indicate the sequencing. 19. Main term is “anemia”. 20. Main term is “diabetes” and this is a diabetes with unspecified diabetic retinopathy which is included in the correct code. Make sure you use the Coding Tips document for number of codes for each question and tips for translating the documentation.
