Use the template provide below and follow the instructions. Template 1 -Toolbox Talk Plan Item Notes (20 – 30 words/item) A Description of the hazard Physical hazards are those substances which threaten your physical safety. The most common types of physical hazard BSBWHS411 - Implement and monitor WHS policies, procedures and programs V3 2021 Page 10 RTO Provider: 91153 - CRICOS Code: 02672K greenwichcollege. edu.au
Item Notes (20 – 30 words/item) are: Fire Explosion Chemical Reactivity Are factors within the environment that can harm the body without necessarily touching it. Physical Hazards include: Radiation: including ionizing, nonionizing (EMF’s, microwaves, radiowaves, etc.) High exposure to sunlight/ultraviolet rays Temperature extremes – hot and cold Constant loud noise B Causes of the hazard Kitchen stores Smoking Long/ Unkempt Hair C Effects of the hazard Kitchen Stoves, make the fire extension bigger in area Smoking, could breath in though dead when got too smoky Long/ Unkempt Hair, could harm the worker D Control methods for the hazard (using the hierarchy of control) E The importance of consultation in identifying and treating hazards in the workplace Consultation is defined in the Occupational Health and Safety Act 2000 as the sharing of relevant information on occupational health and safety where employees are given the opportunity to express their views and contribute in a timely fashion to resolution of occupational health and safety issues. Item Notes (20 – 30 words/item) A health and safety committee can be a good option if you have a significant number of workers who may be reluctant to take on the role of a health and safety representative (HSR), but some would be willing to participate on a committee. Committee meetings bring together employees' hands-on experience of the job and the employer's perspective of the workplace and business requirements. They provide a regular forum for collaboratively developing and reviewing health and safety systems and addressing ongoing and emerging concerns. F An explanation of the relevant provisions of the legislation (such as work health safety legislation), regulations or codes of practice that may apply to the hazard and note when you checked these and when you plan to next check them in order to keep up-to-date with changes in legislation An overview of the organisation’s WHS policies, procedures and programs. The Work Health and Safety Act 2011 (the WHS Act) provides a framework to protect the health, safety and welfare of all workers at work. It also protects the health and safety of all other people who might be affected by the work. The WHS Act also provides protection for the general public so that their health and safety is not placed at risk by work activities. The WHS Act places the primary health and safety duty on a person conducting a business or undertaking (PCBU). The PCBU must ensure, so far as is reasonably practicable, the health and safety of workers at the workplace. Duties are also placed on officers of a PCBU, workers and other persons at a workplace. The WHS Act 2011 outlines the duties of officers of a PCBU. Officers are defined in the Act as a person who makes decisions, or participates in making decisions, that affect the whole, or a substantial part, of a business or undertaking and has the capacity to significantly affect the financial standing of the business or undertaking. If a person is responsible only for implementing those decisions, they are not considered an officer. It is an officer’s duty to exercise due diligence to ensure their business or undertaking fulfils its health and safety obligations under the Work Health and Safety Act 2011. The essential elements of due diligence for an officer are interrelated and cumulative in nature. These elements require an officer: - to acquire and keep up to date knowledge of work health and safety matters BSBWHS411 - Implement and monitor WHS policies, procedures and programs V3 2021 Page 12 RTO Provider: 91153 - CRICOS Code: 02672K greenwichcollege. edu.au

Item Notes (20 – 30 words/item) - to gain an understanding of the operations of the business and the hazards and risks involved - to ensure appropriate resources and processes are provided to enable hazards to be identified and risks to be eliminated or minimized - to ensure information regarding incidents, hazards and risks is received and the information is responded to in a timely way - to ensure the PCBU has, and implements, processes for complying with any legal duty or obligation - to ensure processes are verified, monitored and reviewed. G How you plan to follow up to consult again on hazard and treatment I would use the hazards and associated risk in my office as the tax accountant. The hazard would be the environment in the office, anything that harm people. A risk arises when it's possible that a hazard will actually cause harm. The level of risk will depend on factors such as how often the job is done, the number of workers involved and how serious any injuries that result could be. H A description of where workgroup members can access information about the hazard and where they can access information about information about Australian Hardware’s WHS policies, procedures and programs WHS procedures will then give further specific instructions on how to undertake tasks in a way that is safety focused. Each procedure needs to be written for the conditions on site. The policy and procedur stored in sharepoint that all employee can access. The WHS procedures for your organisation may cover: Acquisition, use, storage and disposal of hazardous chemicals. Consultation arrangements for workers in work area. Alcohol and other drug intoxication. Hazard reporting procedures. Safe operating procedures and instructions. Maintenance and use of plant and equipment. Procedures for hazard identification Purchasing. Procedures for risk assessment, and selection and implementation of risk controls. Workplaces can be dangerous; there are many hazards that have the potential to kill, injure or cause ill health or disease. Protecting the health and safety of people in the workplace is a community expectation that makes good business sense. This Standard Operating Procedure should be the base for the everyday training programme of every employee. The Standard Operating Procedure should be often updated to insure of obedience to the realisation conditions and the working practise. Team leader has obligations under the Occupational Health and Safety Act 2004 (OHS Act) to ensure the health and safety of people ‘so far as reasonably practicable’. Training procedure Step 1 – Identify hazards Identifying hazards involves finding all of the foreseeable hazards in the workplace and understanding the possible harm that the hazards may cause. Mechanical hazards - Plant, equipment and items (and parts of them) that have the potential to cut, rip, tear, abrade, crush, penetrate, produce projectiles or cause sudden impact. Methods for identifying hazards - Inspecting the workplace - Testing and measuring - Surveys of employees and others at the workplace - Analysing records and data Step 2 – Assess risks Risk assessment is a process for developing knowledge and understanding about hazards and risks so that sound decisions can be taken about control. Step 3 – Control hazards and risks Leader of team is required to ensure health and safety by controlling risks. Risks must be controlled by eliminating them so far as reasonably practicable or, if this is not possible, reducing the risks that remain so far as reasonably practicable. Step 4 – Check controls Controls that are put in place to protect the health and safety of people need to be monitored to ensure that they work as planned. This requires checking them and ensuring that processes are put in place to identify and quickly fix problems. Checking controls Checking controls involves the same methods as in the initial hazard identification step (step 1), and creates the loop in which workplace health and safety measures are maintained. Common methods used to check the effectiveness of controls are: - inspecting the workplace - consulting employees - testing and measuring - using available information - analysing records and data. Appendix 1 – Scenario You are employed as a team leader of a group of five administration staff in the head office of Australian Hardware. A rapid growth in the organisation has seen many new computers installed in the office. With so many computers around the office, power extension cables occasionally become loose and end up lying on the floor of the hallway. In the past three months, a number of your employees have had near-misses where they have almost tripped on a loose cable. To make matters worse, just this morning you witnessed your senior administrator, Rosie Thompson, trip on a cable and sprain her wrist on impact with the floor. As team leader, you immediately gave Rosie some ice and a bandage and took her to see the local doctor. Organisational recordkeeping and consultative procedures, which are in place to ensure the organisation adheres to WHS obligations under relevant legislation, require team leaders to: ● conduct regular workplace inspections and submit safety inspection forms to the relevant person (your health and safety representative or your assessor) ● fill in and submit incident reports for incidents involving accidents or near- accidents involving staff or visitors ● compile WHS aggregate information and report on safety performance as part of the WHS management system to your health and safety representative. In your workplace’s operational plan, the target number of incidents or injuries requiring first aid and/or medical treatment is zero. Hazard Identification Procedure WHS legislation requires that PCBUs, in consultation with workers identify all potentially hazardous things or situations that may cause harm. In general, hazards are likely to be found in the following; - Physical work environment, - Equipment, materials or substances used, - Work tasks and how they are performed, - Work design and management In order to identify hazards, complete the following steps: (i) Past incidents/accidents are examined to see what happened and whether the incident/accident could occur again. (ii) Employees consulted to find out what they consider are safety issues, I.e. ask workers about hazards/ near misses they have encountered as part of their work. Sometimes a survey or questionnaire can assist workers to provide information about workplace hazards. (iii) Work areas or work sites be inspected or examined to find out what is happening now. Identified hazards should be documented to allow further action. The work environment, tools and equipment as well as tasks and procedures should be examined for risks to WHS. (iv) Information about equipment (e.g. plant, operating instructions) and Material Safety Data Sheets be reviewed to determine relevant safety precautions. (v) Welcome creative thinking about what could go wrong, i.e. what hazardous event could take place here? Any hazard which is identified by this process should be recorded on the Risk Assessment and Control Sheet and further action taken to assess and then control the risks from this hazard. WHS Consultation Procedure Overview Consultation is mandatory under the Work Health and Safety Act 2011 (WHS Act) for Australian Hardware. Consultation will take place on all work health and safety matters including: undertaking risk management activities proposing changes that may affect workers making decisions about any work health and safety procedures the adequacy of facilities for the welfare of workers. This procedure should be read with the Work Health and Safety Policy. Procedure Australian Hardware consults, so far as is reasonably practicable, with workers who carry out work for Australian Hardware who are (or are likely to be) directly affected by a work health and safety matter. If the workers are represented by a health and safety representative, the consultation will involve that representative. Consultation at Australian Hardware involves sharing of information, giving workers a reasonable opportunity to express views, and taking those views into account before making decisions on health and safety matters. 1 Health and safety representatives (HSRs) HSRs will be elected to represent various work groups and investigate health and safety issues. 2 Health and Safety Committee (HSC) An HSC will be established on the initiative of Australian Hardware. The role of the HSC is to facilitate cooperation between Australian Hardware and workers on health and safety matters as well as to develop work health and safety standards, rules and procedures. 3 Other Agreed Arrangements BSBWHS411 - Implement and monitor WHS policies, procedures and programs V3 2021 Page 29 RTO Provider: 91153 - CRICOS Code: 02672K greenwichcollege. edu.au

Other agreed arrangements for consultation may be set up by Australian Hardware provided the arrangement is agreed to by the workers and is likely to result in better consultation and improved decision making. Once established, consultation must be in accordance with the procedures agreed to between Australian Hardware and the workers. Regardless of the arrangement, the obligation of Australian Hardware to consult with all workers remains. Australian Hardware may need a mix of HSRs, HSCs and / or other agreed consultation arrangements tailored to suit the workers and Australian Hardware’s work environment.
