Successful completion of this assessment task requires you to demonstrate your skills and knowledge by working with three real individuals and using their individualised plans as the basis for support provision. You must:

provide support services
monitor support services
complete recording and documentation. For the purposes of this assessment task, you will require access to Simulation Pack on Moodle
. You will also require access to the relevant policies and procedures.
2.
Complete your project.
There are two sections that need to be completed:
Section 1– Student evidence You are required to fill in answers to all the questions. One of these must be completed for each
of the three individuals.
Section 2 – Critical reflections You need to complete this section after you finish Section 1. You must make sure you receive permission to include any documents from your workplace and all private information must be redacted:
