MOC GROUP PROJECT
Purpose:
The purpose of this assignment is to promote knowledge of community resources so you can advocate, educate, and provide anticipatory guidance for your patient(s) and their families.
Community Agencies/ Resources
Your group will research and identify 3-4 (depending whether you’re a group of 3 or 4) local agencies that are representative of your group topic.
Once you have identified your resources, please send the identified resources to your professor for approval (I want to avoid duplication of resources). Multiple groups will not be approved to use same agencies as the goal is to help each of you become aware of available resources within the community. Once approved, you are good to go!
All team members must visit each agency—I would suggest calling to make an appointment. A picture of your group must be taken at each agency, and be included in your presentation. Talk
with someone at the agency to gain an understanding of what is offered, who it is offered to, how to access and how the agency works for the community. Please refer to the rubric for specific project requirements. I’d also like you to cover how Covid-19 may have changed how the agency does business.
LOG # 1 and Log #2
The logs are meant as a check in. They are not worth points but must be submitted to demonstrate timely completion. Log #1 has a due date (see syllabus) and Log#2 is turned in with the final presentation. Please have one person upload the logs into the drop box by assigned due dates listed in modules and the “at-a-glance.”
5 Evidence-Based References
Your group will need to identify 5 Evidence-Based References that relate to your patient population, you will complete a reference page in the latest APA format. One person will upload the evidence-based references to the drop box by the assigned due date. See syllabus for due dates. These references must be in APA format. These references are not worth points but must be submitted to demonstrate timely completion.
Project Presentation
The information you obtain at your agencies will be turned into a power point presentation per the presentation rubric in the group project module. Presentation dates will be scheduled in week 5 and presentations occur weeks 11-14. Please refer to the rubric for specific requirements. Presentation times/date will decided in class.
