SED 104 Individualized Education Program (IEP) Module (50 points) Purpose: Teachers must be aware and knowledgeable of the Individualized Education Plan document and process. Directions: Fill in the answers to the following questions and submit it to Blackboard once it is completed. You should use the resources on the IEP Module to complete this assignment. **Feel free to take any other notes or write in any questions you may have about the material. IEP Questions Answer the prompts in this column Overview (5 points) (1 pt) What does IEP stand for? Individualized Education Program (2 pt) How often are IEPs written and reviewed? Any student from age 3-21 that has a disability or handicap that impedes their learning (2 pt) Who would be on a student’s IEP (in KY we call this the ARC) team? Include at least 4 people that should be on this team/committee It can be a range of people including the parent or legal guardian, General and Special Educators, Administrators, and in the best of cases the student. Present Levels (4 points) (4 pt) In Present Levels, what information should be included here? It is used as a starting line of measurable information to compare to as the student progresses through their education. It helps to develop and achieve goals. It is how the teaching would be differently designed to help the student learn and achieve more (3 pt) What are supplementary aids and services (SAS)? Give an example of SAS. These are aids or services that are there to support a student in a general education classroom and other classroom related settings. (3 pt) What are accommodations? Give examples of accommodations. a comparison of the student's performance using an accommodation and without using an accommodation (Example: a student answers 11 out of 15 questions correctly on a grade-level text with a reader. The Student then answers 2 out of 15 questions correctly without a reader on a grade-level text.) (3 pt) What are modifications? Give examples of modifications. They are provided in order to enable a child to do all things listed in IDEA’s provisions. Modifications include the use of school time and use of school staff. (Example: a supervised environment for addressing individual health needs, or school staff will minimize distractions for the student.) (2 pt) what is least restrictive environment (LRE) and how should it be recorded in an IEP? An environment that is least obstructive but provides the right amount of support for a student to succeed. The LRE is documented on the IEP. (2 pt) How do students qualify for extended school year services? A student must loose of the skills that they learned in the school year.

Include the age according to IDEA (federal requirements) and the age/grade listed in KY requirements (note: KY has chosen a different age than is required in federal regulations, so you need to include BOTH ages- to find the KY age, look at the KY IEP) Any special education student that has an IEP and in the United States must begin at 16 and in Kentucky its for anyone ages 3-21. (2 pt) What types of services must be included in transition services? In what document are transition services listed? It outlines the students transition goals and services for the student. The services must be listed inside the IEP. The IEP Meeting (10 points) Watch an example of a middle school IEP meeting at the following link. At the start of the meeting every person introduced themselves 2. The student is a part of the IEP process. This gives them a voice and allows them to voice their concerns. 3. Parents concerns were a priority and they did not feel left out of the process 4. Time outside of the classroom like lunch and class changes are taken into consideration in the IEP 5.
