MODULE FIVE
5-3 Journal: Final Project Case Study
How I determine:
which codes are ICD, CPT or HCPC II
First, I had to understand the difference between the types of coding systems.
ICD-10 codes are used to identify the diagnosis, or the reason the patient is seeking.
treatment from the healthcare professional.
CPT codes are used for what is done to the patient - diagnostics, labs, radiology and
surgical procedures etc.
HCPCS are used to report non-physician services such as ambulance rides, durable.
medical equipment, supplies and medications.
I read the entire chart - beginning with the face sheet or registration. I review the
patient's history, reason for being seen and where they are being seen. I read any special
reports, E.g. - operative or special procedure. I review any test reports like labs or
radiology, progress notes, physician's orders and medications. I then determine main.
terms and key words to narrow down which coding systems to use first.
Main terms
in the patient record:
Acute and chronic alcoholism, chest x-ray,
left wrist x-ray, doing a review of the patient records, below I listed some of the main terms that I will be researching in Select Coder
Alcoholism
2.Radiologic chest
3.Radiologic wrist hand
4.Emergency Exam
5.Alcohol counseling
6.Surgery Consult
7.Cirrhosis
8.CBC
9.Urinalysis
10. Electrolytes
11. Valium
12. Thiamine
13. Teragram
14. Tylenol
15.Radiologist (consult)
16.Blood Chemistry (Basic Metabolic Panel)
17.Fracture left Wrist
Terms:
for the most part, I do understand the codes and how they should be used, I do feel that some of my struggles are the main terms to use when searching. Radiological procedures, but I feel I have been doing better as I complete each week's assignments.
I understand what ICD-10 is used for - but have some trouble differentiating between
CPT and HCPCS. It seems like there is a lot of back and forth between the two coding systems.
But with practice and time and more use of the system it will make more sense I’m hoping. The good thing is it defiantly challenges the brain.
Question
– Is there a particular order the ICD-
10, CPT then HCPCS? Go in?
