Community Assessment Plan: Part A, Outline
A community assessment to determine the health status of individuals is determined by a
nurse. Using the Gordon’s functional health patterns, a complete community assessment will be
obtained from the city of Pomona to collect information that identifies problems in the
community (Edelman & Kudzma, 2022, p. 147). The purpose of the Pomona Community Health
Needs Assessment is to collect information within the community and analyze the needs of the
community based on what they lack for health promotion. After the assessment, information and
education will be provided to the Pomona community about the health of the population, and
resources for health care access, can be made available to the population.
Community Assessment Resources
Significant community needs were found based of the findings on the community
assessment. Based off a 2021 study performed by Pomona Valley Hospital Medical Center the
highlights of findings with the biggest needs are access to affordable health care, prevention
services and screening, mental health services, and community wide partnerships. The analysis
of primary and secondary data let to the conclusion that the health care needs from the
community were important. They were ranked as highly and largely significant and if given
resources there would be significant changes in community health improvement.
The highly significant needs are needed on minority populations and vulnerable
populations. Affordable primary care, prevention services and screenings are mostly needed for
low-income populations, homeless, minority groups such as undocumented populations, seniors,
LGBTQ+, and communities of color (2021). Mental health services are strongly needed for the
homeless population, as well as for uninsured, youth, and people with severe mental illness.
Community wide partnerships services are needed for seniors, low-income communities, patients

with special needs, and again homeless and undocumented communities. Nevertheless, the
populations that need the resources the most are people with chronic disease such as
hypertension, diabetes, obesity, and cardiovascular illness.
Community Strengths and Concerns
Every community with significant community needs also counts with the community
strengths and concerns. The community may count with certain resources, but it is unused
because the public does not know about the resource. The community nurse must make
judgement about the community health and how the community responds to the health situations
(Edelman & Kudzma, 2022, p. 218). With that said, even though there is a developed resource
for the community, many time the concern is that the resource is unavailable, inaccessible, or
dysfunctional. At that point the resource is worthless because the community has no use of it.
The community strengths for the city of Pomona are available for the public. According
to the
Community and field services division
, the city of Pomona count with a wellness center
which offers mental health services, substance abuse, and wellness information (2023). The
concern with this community strength is the lack of knowledge for the community, many people
don’t know about this wellness center. Community health fair needs to be offered with pamphlets
to advertise the services provided with this wellness center. Another strength in the community is
sex education in schools, although this is a strength, many parents lack communication with their
children regarding sex and not much attention is given to this subject.
Potential Barriers
Trying to implement community health sources can have some barriers. Barriers such as
financial issues, lack of communication, lack of access to health care, lack of patient health
literacy. The barriers on trying to implement health sources can be difficult for people to get

access to the resources being provided. There needs to be ways to address the barriers in order to
improve community healthcare.
The first barrier is the cost and financial issues, for example inability to pay for health
insurance and lack of transportation for health appointments (2021). This barrier can be
addressed by providing low-cost insurance and providing people with resources on how to apply
and if needed help to fill out applications. Another barrier is lack of communication between the
community and their provider, many times is due to language barriers (2021). This barrier can be
addressed with translator phones making sure the patients fully understand what the provider is
recommending. Lack of access to healthcare is another barrier, many people do not have access
to health care provider due to not enough providers in the area or their lack of understanding on
how to access the healthcare system. The most important barrier is lack of patient health literacy,
many don’t understand the need of routine checkup appointments and screenings which leads to
chronic diseases, this barrier can be overcome by providing education to the communities,
detailed explanation on the importance of receiving health care.
Conclusion
In conclusion, performing a community assessment is very essential to analyze the
strengths of the community and what the community needs to improve health promotion.
Barriers to community resources need to be overcome by addressing the barrier and change.
More community education needs to be given, health care fairs in different parts of the
community with pamphlets in different languages need to be provided. Providing the community
with what they need will result in positive outcomes and less chronic illness.
